Type 2 Heart Attack:

When work load on heart muscle increases despite restricted or limited blood/oxygen supply causing demand/supply imbalance, heart muscle death ensues.

Almost 1/3 to 1/2 of all heart attack patients presenting to Emergency department

Precipitants:
Hypertension - load on heart muscle
Abnormally high heart rate - increased demand
Anemia - decreased oxygen supply
Surgery - increased demand
Heart failure - weak heart muscles
Infection - increased demand/decreased supply
Severe low BP

More chances of death if not picked up early.

Heart awareness in easy language

Type 1 Heart Attack: Acute coronary syndrome

Immediate cause: Sudden blocking of blood supply to heart muscles due to clot formation

Underlying cause: Altered fat deposits in blood vessels causing narrowing of lumen

Consequences: Heart Muscle death leading to Pump failure and electric currents in heart go haywire.

Symptoms: Muscle death leads to chest painPump failure leads to back pressure in lungs causing Breathlessness (not the only cause)Disorderly currents lead to loss of contraction of normal remaining muscles→no blood to brain→Loss of consciousness/fainting/death

Treatment: Restore blood supply to heart muscles by;
1. Dissolving clot by urgent medication called Thrombolysis
2. Putting in stent (artificial tube) to remove obstruction
3. Bypassing obstruction by planting new blood vessel

10/01/2024

Rheumatoid arthritis is a type of autoimmune disease primarily involving joints.

Small joints of Hand and/or other joints get swollen and red, painful to move and touch.

Eyes, lungs, spine, nerves, skin and kidneys can also get involved.

Debilitating disability with crazy bent joints and inability to do basic life skills.

Treatment for most of them is easy and relief is quick. However, despite relief, disease doesn't magically vanish, it is still there and controlled or dormant at times.

Name 'Rheumatoid' means Rheumatic like. Rheumatic joints until recently were joint pain seen in Rheumatic fever, which is an immune sequelae of mis/untreated streptococcal throat infection.

Autoimmune diseases are a group of diseases in which the Immune System gets programmed to attack the body's own tissues leading to inflammation.

Every normal fetal/child immune development involves recognition of 'Own' vs 'Other' tissues, usually in the Thymus gland. And the loss of a part of this tolerance to the body's own tissues leading to mounting an attack on them causes disease.

Skin, joints, kidneys, heart, nerves are commonly affected.

Most common of them are;
Rheumatoid arthritis
SLE (called Lupus- from resemblance to wolf bite)
Sjogrens syndrome
Celiac disease

Many of them are organ and life threatening if not diagnosed and treated properly.

Streptococcal infections (a type of bacteria) of throat or skin can involve kidney via immunological phenomenon called Postinfectious Glomerulonephritis

Develops a few weeks after infection

Presents with swelling of body/face, high BP, decrease urine output, protein and blood loss in urine, which if progresses unchecked can lead to need for dialysis and transplant.

So, via few scoring systems available and rapid antigen test, we should diagnose and treat streptococcal infections with appropriate dose and duration of antibiotics.
