A skin assessment is a systematic examination of the skin to evaluate its health, identify abnormalities, and detect early signs of diseases. It involves inspecting and palpating the skin to assess various characteristics.
Key Components of Skin Assessment:
1. Color β Check for pallor, cyanosis, jaundice, redness, or changes in pigmentation.
2. Temperature β Assess warmth or coolness using the back of the hand.
3. Moisture β Determine if the skin is dry, moist, or excessively sweaty.
4. Texture β Evaluate smoothness, roughness, or thickness.
5. Turgor & Elasticity β Test for dehydration by pinching the skin and observing how quickly it returns to normal.
6. Lesions & Rashes β Look for moles, ulcers, wounds, or any abnormal skin growths.
7. Edema (Swelling) β Check for fluid retention by pressing the skin and observing indentation.
8. Integrity β Identify wounds, pressure ulcers, or areas at risk of breakdown.
9. Hair & Nails β Assess for signs of malnutrition, infection, or systemic disease
