દક્ષિણ ગુજરાતમાં પ્રથમવાર માઇક્રોવેવ પદ્ધતિથી લિવર ના કેન્સરની ગાંઠ ની સારવાર

ગુજરાતની સૌથી આધુનિક એવી સુરતની કિરણ મલ્ટી સ્પેશીયાલીટી હોસ્પિટલ માં ઇન્ટરવેન્શનલ રેડીઓલોજિસ્ટ ડો. જીગર આઈયા અને ડો. પરેશ પટેલ દ્વારા લિવર ની કેન્સરની ગાંઠોનું જટિલ ઓપરેશન માઇક્રોવેવ પદ્ધતિથી કાપકૂપ વગર સળતાપૂર્વક કર્યું. માઇક્રોવેવ પદ્ધતિ થી આ પ્રકારનું ઓપરેશન દક્ષિણ ગુજરાતમાં પ્રથમ વાર થયું છે.

૬૫ વર્ષના એક દર્દીને મળાશય નું કેન્સર હતું જે પેટમાં ફેલાઈ ગયું હતું જેનું મોટું જટિલ ઓપરેશન બે તબક્કામાં સફળાપૂર્વક થયું હતું અને તે પછી દર્દી એ કેમોથેરાપી પણ લીધી હતી. સમયાંતરે તપાસ કરતા ખબર પડી કે લિવર માં બે ગાંઠો આવી છે જેની બાયોપ્સી કરતા ખબર પડી કે મળાશય ના કેન્સરે ઉથલો માર્યો છે જેની ગાંઠો હવે લિવરમાં આવી છે.

આ ગાંઠોની સારવાર કરવા મોટું ઓપરેશન કરી લિવરની ગાંઠો વાળો ભાગ કાઢી નાખી ને ફરીથી કેમોથેરાપી લેવાની થાય, પરંતુ પહેલા બે વાર ઓપરેશન થયા હોવાથી ફરીથી મોટું ઓપરેશન કરવું બહુ જોખમી છે વળી એમાં નોર્મલ લિવરનું ઘણું ભાગ પણ નીકળી જાય.

આ પ્રકારની કેન્સરની ગાંઠો જે પાંચ સે.મી. થી નાની હોય ને સંખ્યામાં પાંચ કરતા ઓછી હોય તો થર્મલ એબ્લેશન (RFA/MICROWAVE) એટલે કે નાની સોઈ દ્વારા ટાંકા વગરના ઓપરેશનથી બાળી શકાય છે. કેન્સરની બે ગાંઠોમાંથી મોટી ગાંઠ લિવરની મોટી લોહીની નળીઓ ની ખૂબ જ નજદીક હોવાથી આધુનિક માઇક્રોવેવ (Microwave) પદ્ધતિ દ્વારા સારવાર કરવામાં આવી. આ પદ્ધતિમાં નાની સોઈ ને ગાંઠની અંદર નાખવામાં આવે છે ને પછી ઊંચા તાપમાને ગાંઠને બાળવામાં આવે છે જેથી કેન્સરના કોષો બળી ને નિષ્ક્રિય થઈ જાય.

આ પ્રકારની સારવારથી ઓછા જોખમે, કાપકૂપ વગર થાય છે અને દર્દીને ઓપરેશન કર્યાના બીજા દિવસે જ હોસ્પિટલ થી રજા મળી જાય છે.

ઇન્ટરવેન્શનલ રેડીઓલોજી એ મોડર્ન મેડિસન ની આધુનિક શાખા છે. જેમાં પગથી લઇ મગજ સુધીના દરેક અંગોની તપાસ કે સારવાર થઈ શકે છે. જેમ કે જટિલ બાયોપ્સી, શરીરમાં ભરાયેલું પાણી કે પરું કાઢવું, શરીરમાં કોઈ પણ જગ્યા એથી લોહી નીકળતું હોય તો એને બંધ કરવાનું કે લોહી જામી ગયું હોય તો એ કાઢવાની સારવાર, વેરિકોઝ વેઇનની લેઝર થી સારવાર, પગમાં થતાં ગેંગ્રીન ની સારવાર, લિવરના કેન્સરની સારવાર, બચ્ચાદાની ના ગાંઠ (ફાઈબ્રોડ) ની સારવાર, મગજ કે કરોડરજ્જુની લોહીની નળીઓના ગુચ્છા કે લોહીના નળીમાં થતાં ફુગ્ગા ને બંધ કરવાની સારવાર, લકવાની સારવાર. આ બધી સારવાર કોઈ પણ જાતના કાપકૂપ વગર થઈ જાય છે.

આ બધી સારવાર કિરણ મલ્ટી-સ્પેશીયાલીટી હોસ્પિટલ માં ૨૪ x ૭ થાય છે અને એ માટે એના સ્પેશીયાલીસ્ટ ડોક્ટરસની ટીમ પણ હાજર છે.

30 Years old Male with Acute Pancreatitis presented with Sudden Onset Worsening of Abdominal Pain. On CT Scan Large hematoma seen in the region of pancreatic Head. On DSA, 2 aneurysms seen *1 on GDA and another on IMA+ Celiac Collateral* with Narrowing of Celiac artery origin So reversal of Flow in GDA & Collateral. Embolisation of Both aneurysms done with Coiling across the aneurysms. - Post Embolisation Complete obliteration of aneurysms with forward flow in Common hepatic artery

૩૦ વર્ષનાં એક દર્દીને દારૂ પીવાથી સ્વાદુપિંડ પર સોજો આવી ગયું હતું. એ સોજાને લીધે આજુબાજુની લોહીની નળીઓમાં ફુગ્ગા બની ગયા હતા જે ફૂટવાથી પેટમાં લોહી નીકળવા લાગ્યું વળી એ નળી સાંકળી હતી જેથી ફુગ્ગા સુધી પહોંચી બંધ કરવું મુશ્કેલ હતું. આ જટિલ ઓપરેશન કોઈ પણ જાતના કોમ્પ્લિકેશન કે કાપકૂપ વગર સફળાપૂર્વક પૂરું પાડ્યું.

Middle aged male with Liver Cirrhosis and Single HCC of 6 cm sized - Not Fit for Surgery. Selective Celiac artery angiogram shows Hypervascular lesion in Right lobe of liver, After Superselective cannulation of feeding artery, Chemoembolisation done. Good deposition of Lipiodol and Chemotherapy drug seen. No new enhancing area seen at 1 month follow up.

ખરાબ લીવર માં ૬ સે.મી.ની કેન્સરની ગાંઠ બની જેની સારવારમાં મોટું ઓપરેશન કરી એટલું લીવર કાઢી નાખવામાં આવે છે. દર્દી મોટાં ઓપરેશન માટે ફિટ નહોતું એટલે કેન્સરની ગાંઠ સુધી જઈ માત્ર ગાંઠમાં જ કિમોથેરાપી આપી. એક મહિના પછી તપાસ કરતા એ ગાંઠ પૂરેપૂરી કન્ટ્રોલ માં આવી ગઈ હતી.

Case - 9 Final case of Thrombotic Complication

53 વર્ષના બેનને અચાનક બેભાન થઇ ગયા. તપાસ કરતા નાના મગજ ને લોહી પહોંચાડતી નળી બંધ થઈ ગઈ હતી. આ પ્રકારના કિસ્સામાં દર્દીના સગા મોડું કરે કે નિદાન થવામાં મોડું થાય તો દર્દી કોમાં માં જતો રહે અથવા મૃત્યું પામે છે. મગજની નળીઓમાંથી લોહીના ગઠ્ઠા કાઢવાની સ્પેશિયલ સ્ટેન્ટ થી લોહીનું ગઠ્ઠો ખેંચી કાઢ્યું.

Middle aged Lady with Covid-19 positive presented with sudden loss of consciousness. On MRI, Small infarct in brain stem (midbrain) with distal basilar occlusion. Mechanical Thrombectoy done and basilar artery recanalised.

Time to unleash the aneurysm series which we did in lockdown.

Case 8 Last case of aneurysm done on last day of lockdown

Elderly male patient presented with severe headache followed by altered sensorium. On CTA, grade 3 SAH with large tubular wide neck aneurysm of right ACA confirmed with DSA. Endovascular Coiling done.

Challenges of coiling
Tortous arteries
Difficult angle of neck of aneurysm
Relatives want less invasive & relative safe treatment option but financial constraints

Endovascular Coiling done successfully and patient's GCS 15/15, no neurodeficit on 3rd post operative day with complaint of mild headache. Discharged on 10th day with uneventful stay in hospital.

Patient is asymptomatic on 2 months follow up.

મોટી ઉમરના આ દર્દીને અચાનક જિંદગીમાં ક્યારેય ના દુખ્યું એવું માથું દુઃખવાનું અચાનક શરૂ થઈ ગયું ને ૩-૪ કલાક માં તો દર્દી બેહોશ થઈ ગયો. આ દર્દીને મગજમાં લોહીની નળી નો ફુગ્ગો ફૂટવાથી મગજમાં લોહી ભરાઈ ગયું હતું. આ દર્દીને ને પગની નળીમાંથી જઈ મગજની નળીના ફુગ્ગામાં coils નાખી ફુગ્ગા ને બંધ કરી દીધું જેથી એ ફુગ્ગો ફરીથી ના ફૂટે. દર્દી 3 દિવસ માં એકદમ બરાબર થઈ ગયો ને રજા વખતે પોતાના પગે ચાલીને ઘરે ગયો.

(મગજમાં લોહી વહેવાના ઘણા કારણોમાં ના એક એવા મગજ ની નળી માં થતાં ફુગ્ગાની સારવાર એકદમ સુરક્ષિત રીતે મગજ ને ખોલ્યા વગર કે કોઈ પણ જાતના કાપકૂપ વગર થઈ શકે છે.)

Case 7

Patient presented with acute onset abdominal pain, on CTA dissecting aneurysm in distal SMA with edematous proximal ilial loop - ? Ischemic
Stent graft placed in dissected artery across the neck of aneurysm.
Pain subside in 1 day.. on follow CT after 5 days - Patent SMA & Bowel edema resolved.

દર્દીને અચાનક પેટ માં દુખાવો ઉપડતા સિટી સ્કેન કરવામાં આવ્યું જેમાં આંતરડાંની લોહી ની નળી ખરાબ થઈ ગઈ ને એમાં ફુગ્ગો પણ બની ગયો.. નળી ખરાબ થતાં આંતરડાને પૂરતો લોહી મળતું નહોતું. પડદા વાળું સ્ટેન્ટ બેસાડી એની સારવાર કરતા દર્દીને ને દુખાવામાં રાહત થઈ ને 5 દિવસ માં એકદમ બરાબર થઈ ગયો.

Case 5

K/c/o Ca. Tongue with recurrent oral bleeding. 1st CT Angiography was normal. Repeat CT Angio was done at the time of bleeding - Small aneurysm in Tumor bed on branch of lingual artery. Posted for embolisation, very difficult to find DSA on comparison the branch on CTA, faint aneurysm seen on DSA. Embolisation done with Glue.

No episodes of re-bleeding for more than 2 months.

જીભ ના કેન્સર ના દર્દીને ને કેન્સરની ગાંઠ માંથી વારંવાર લોહી નીકળતું હતું એટલે જ્યાંથી લોહી નીકળતું હતું એ જગ્યાની નળી બંધ કરી દીધી. હવે છેલ્લા 2 મહિનાથી લોહી નહિ નીકળ્યું.

Case 6

K/c/o acute pancreatitis, presented with hemetemesis. On CTA proper hepatic artery aneurysm. Posted for stent graft placement. On DSA, one more aneurysm seen in Gastroduodenal artery (GDA). So coiling of GDA was done & Stent Graft was placed in Hepatic artery

સ્વાદુપિંડ ના સોજા વાળા દર્દીને ને અચાનક લોહીની ઉલ્ટીઓ શરૂ થઈ ગઈ. તપાસ કરતા 2 લોહી ની નળીઓ માં ફુગ્ગા બન્યા તા જે કોઈ પણ જાતની કાપકૂપ વગર બંધ કરી દીધા જેથી ઉલ્ટી માં લોહી આવવાનું બંધ થઈ ગયું.

Case 3 Stubborn aneurysm vs Stubborn IRs

Pancreatitis with 8-10 mm sized ruptured aneurysm on small pancreatic branch of splenic artery close to its origin.

Plan A - Cannulate pancreatic branch and coil the aneurysm - couldn't cannulate even with neuro Microcatheter & Wire

Plan B - Coil the splenic artery just distal to origin of aneurysm and glue embolisation to occlude proximal splenic artery but after sometimes aneurysm started filling from side of the glue cast!!! 😳 Never seen or expected.

Plan C - Put the stent graft in Celiac artery across the Splenic artery - Stent graft not available in Surat & difficult to get due to lockdown + Financial constrain

Plan D - Percutaneous Glue embolisation
Challanges
1. To identify the 8-10mm sized aneurysm in pancreatic bed with lot of bowel gas.
2. To puncture the aneurysm of 8 mm size through transgastric aproach
3. Chances of needle may come out of aneurysm with respiration.

We faced all problems and succeeded after multiple puncture. 🤗

Case 4 - Not a time to plan or think

Young boy with advanced malignant lesion with ulcerative wound in left side of neck presented with massive bleeding which stopped with compresion - CT angio showed aneurysm in left Common carotid artery at the site of defect. Referred for same. While taking history bleeding - Spurt of blood started in front of us. Patient shifted to cath lab with 1 person was compressing the artery. Dont have time for BTO (balloon test occlusion - to check cross flow) so decided to occlude the artery with risk of paralysis. Occlude the artery with available coils and Sacrifised the artery - Fortunately bleeding stopped no neurodeficit and good collateral to MCA from left P.com A.

30 વરસ ના દર્દીમાં કેન્સર ગળા માં ફેલાઈ ગયું હતું જેનાથી મગજમાં જતી નળી ફાટી ગઇ ને લોહી નો ફુવારો ચાલુ થઈ જતાં એ જગ્યા એ દબાવી તાત્કાલિક કેથ લેબ માં દર્દીને લઈ જઈ ખરાબ થઈ ગયેલી નળી બંધ કરી દીધી.

Case 2

Patient of liver transplant presented with massive hemetemesis. On CTA, psudoaneurysm with severe stensois at anastomotic site of hepatic artery. Limited hardware was available due to lockdown. So we put stent graft of available size and dilated it with bigger balloon. Complete occlusion of aneurysm with dilatation of stensosed segment so improved liver vascularity

(શરીર ની કોઈ ધમની માં કાણું પડે અથવા ફૂટી જાય તો એમાંથી લોહી નીકળે એ આજુબાજુના અંગોથી થોડાક કલાકો કે દિવસો માટે રોકાઈ જાય ને એ જગ્યા એ ફુગ્ગો (psudoaneurysm) બની જાય. એ ફુગ્ગાને જેટલું બને એટલું ઝડપથી બંધ કરવું જરૂરી છે.. જેથી ફરીથી ફૂટે નહિ. આવા ઘણા દર્દીઓની લૉકડાઉન માં સારવાર કરી..)

લીવર ટ્રાન્સપ્લાન્ટ ના દર્દીમાં ટ્રાન્સપ્લાન્ટ કરેલા લીવર ને લોહી પહોંચાડતી નળીમાં ફુગ્ગો બન્યો જેમાં ફુગ્ગો બંધ થઈ જાય ને લીવર ને પૂરતું લોહી પણ મળી શકે એ રીતે પડદા વાળી સ્પ્રિંગ બેસાડી દીધી.

Case 1

Pancreatitis with ruptured giant splenic artery aneurysm - coiling + glue embolisation done. Splenic parenchymal vascularity preserved with collaterals from right gastroepiploic artery.

(શરીર ની કોઈ ધમની માં કાણું પડે અથવા ફૂટી જાય તો એમાંથી લોહી નીકળે એ આજુબાજુના અંગોથી થોડાક કલાકો કે દિવસો માટે રોકાઈ જાય ને એ જગ્યા એ ફુગ્ગો (psudoaneurysm) બની જાય. એ ફુગ્ગાને જેટલું બને એટલું ઝડપથી બંધ કરવું જરૂરી છે.. જેથી ફરીથી ફૂટે નહિ. આવા ઘણા દર્દીઓની લૉકડાઉન માં સારવાર કરી..)

આ દર્દીને વધુ દારૂ પીવાથી સ્વાદુપિંડ માં સોજો આવી ગયું હતું જેથી બરોળ ની ધમની માં ફુગ્ગો બન્યો જે એકવાર ફાટી ગયું હતું પણ નસીબે થોડાક સમય માટે રોકાઈ ગયું હતું જે ફરીથી ફાટે નહિ એ માટે એને ધમની ની અંદર જઈને બંધ કરી દીધું.

Similar to previous case but this time with acute SMA occlusion, on CT ilial loop edema. Catheter directed thrombolysis was done. Post CDT near complete recanalization of SMA. Over the 4 days pain subside and Patient is now on full diet..

આગળના દર્દીની જેમ આ દર્દીને પણ પેટ ની લોહી ની નળી બંધ થઈ ગઈ હતી પણ આ દર્દીને આંતરડાની નળી બંધ થઈ ગઈ હતી જેના લીધે આંતરડા ને લોહી નહોતું મળતું (આંતરડાનું એટેક કઈ શકાય). આવા કેસ માં આંતરડા નું નેક્રોસિસ (આંતરડા નું ભાગ ખરાબ થઈ જવું) થાય તો ૬૦% કેસ માં દર્દીને બચાવી શકાતું નથી એટલે નળીને ઝડપથી ખોલી આંતરડું ખરાબ થતું અટકાવી શકાય છે.

Left thigh Heamangioma with severe pain. Pai score 9 - Embolisation + sclero. Post procedure pain score 0-1

દર્દીને ડાબા થાપા માં લોહી ની નળીઓ ની સાદી ગાંઠ જનમ થી જ હતી પણ થોડાક દિવસથી એમાં દર્દ થતાં એ સારવાર માટે આવ્યા. કાપકૂપ કર્યા વગર અંદર થી નળીઓ બંધ કરી (embolisation) ને ચામડીમાંથી દવા (sclerotherapy) નાખી દર્દીને દર્દ માં પૂરેપૂરી રાહત આપી.

Single ~ 5 cm size HCC. Plan for TACE+ RFA. TACE done with good deposition of Lipiodol in lesion and PV.

લીવર ના કેન્સર માં ડાયરેક્ટ કેન્સર માંજ કેમોથરાપી આપવાથી કેમોથેરાપી ડોઝ ઘણું ઓછું કરી શકાય ને એની આડઅસર શરીર ના બીજા ભાગ માં પણ થતી નથી. જરૂર પડે તો ગાંઠ ને બાળી પણ શકાય છે. આ પ્રકારની સારવાર થી દર્દીને નહિવત આડઅસર સાથે મહત્તમ ફાયદો થાય છે.

Carotid stenting for severe stenosis with unstable plaque. During plasty dislodged and trapped in spider.

દર્દીને વારંવાર લકવા ના એટેક આવતા જે થોડાક સમય માટે રહેતા ને પછી સારું થઈ જતું. તપાસ કરતા મગજમાં જતી ગળાની નળી માં બ્લોક હતું એટલે stent બેસાડી બ્લોક ને ખોલ્યું.

જીભ ના કેન્સર ના લીધે એની ધમની ખરાબ થઈ જવાથી દર્દીને મોઢામાંથી ઘણું લોહી નીકળતા કાપકૂપ વગર એ નળી ને બંધ કરી દર્દીને મોઢામાંથી નીકળતું લોહી બંધ કરી ને દર્દીનો જીવ બચાવ્યું.
