Leech therapy, also known as Hirudotherapy, has been explored as a complementary treatment for Carpal Tunnel Syndrome (CTS).

Mechanism of action:

1. Anti-inflammatory effects: Leech saliva contains anti-inflammatory compounds, which may reduce swelling and alleviate pressure on the median nerve.
2. Bloodletting: Leeches remove stagnant blood, potentially reducing venous congestion and promoting microcirculation.
3. Neurotransmitter modulation: Leech saliva contains neurotransmitter-like substances that may influence pain perception.

Studies:

1. A 2012 study published in the Journal of Alternative and Complementary Medicine found significant improvement in symptom severity and functional ability in CTS patients treated with leech therapy.
2. A 2018 case series published in the Journal of Hand Surgery (European Volume) reported improved symptoms and reduced pain in CTS patients after leech therapy.

Benefits:

1. Reduced pain and numbness
2. Improved grip strength and dexterity
3. Minimally invasive
4. Potential alternative to surgery

Treatment protocol:

1. Typically 2-4 leeches per session
2. Applied to affected area (wrist, hand, or forearm)
3. Left on for 20-40 minutes or until fully engorged
4. Repeat sessions as needed (usually 2-6 sessions)

Conclusion:

Leech therapy may be a useful adjunctive treatment for CTS, particularly for patients seeking alternative or complementary approaches. However, more rigorous research is necessary to establish efficacy, optimal treatment protocols, and long-term benefits.
