💀🦷 Managing a Complex Mandibular Fracture Case! 🦷💀

Mandibular fractures are among the most frequent facial fractures, often caused by trauma, falls, or road traffic accidents. This patient sustained a left angle fracture and a right parasymphysis fracture, leading to severe pain, malocclusion, and restricted jaw movement.

🔹 Fracture Breakdown:
✅ Left Angle Fracture: Typically displaced due to muscle pull, requiring rigid fixation.
✅ Right Parasymphysis Fracture: Can affect occlusion and stability, needing careful alignment.

🔹 Treatment Plan:
✔️ Pre-op Imaging: OPG/CBCT for precise diagnosis
✔️ ORIF (Open Reduction & Internal Fixation): Miniplate and screw fixation for stability
✔️ Occlusion Correction: Intermaxillary fixation (IMF) if necessary
✔️ Post-op Care: Antibiotics, soft diet, and jaw physiotherapy

🔥 Submasseteric Space Infection – A Deep Facial Space Abscess! 🔥

The submasseteric space is located between the masseter muscle and the lateral surface of the mandibular ramus. Infections here usually originate from an infected third molar (especially pericoronitis) or mandibular trauma. Due to the dense fascia and muscle layers, swelling may be localized but extremely painful, with trismus (difficulty opening the mouth) being a key clinical feature.

🔹 Management Approach:
✅ Clinical evaluation & imaging (CBCT helps in locating the abscess)
✅ IV antibiotics & pain management
✅ Surgical drainage via an intraoral or extraoral approach
✅ Removal of the offending tooth (if present)
✅ Warm saline/mouth rinses & physiotherapy for trismus

💡 If left untreated, the infection can spread to deeper spaces, leading to serious complications like Ludwig’s angina or descending necrotizing fasciitis.

🦷💀 Bilateral Mandibular Fractures – A Complex Case! 💀🦷

Mandibular fractures are common facial injuries, often resulting from trauma, road traffic accidents, or physical assault. This patient presented with a left angle fracture and a right parasymphysis fracture, causing severe pain, malocclusion, and difficulty in jaw movement.

🔹 Understanding the Fractures:
✅ Angle Fracture (Left Side): Commonly occurs due to impact at the lateral border of the mandible. Often displaced due to muscle pull.
✅ Parasymphysis Fracture (Right Side): Occurs in the anterior mandible, affecting occlusion and stability.

🔹 Management Plan:
✔️ Preoperative Imaging: OPG/CBCT to assess fracture displacement
✔️ Rigid Fixation: Open Reduction & Internal Fixation (ORIF) with titanium miniplates & screws
✔️ Occlusion Restoration: Intermaxillary fixation (IMF) if required
✔️ Post-op Care: Antibiotics, soft diet, and physiotherapy for jaw function

🚨 Ludwig’s Angina – A Life-Threatening Emergency! 🚨

Ludwig’s Angina is a rapidly progressing, life-threatening cellulitis of the submandibular, sublingual, and submental spaces. It typically arises from an infected mandibular molar and can cause severe swelling, airway obstruction, and systemic toxicity.

🚑 Emergency Management:
✅ Securing the airway (may require intubation or tracheostomy)
✅ IV antibiotics & fluids
✅ Surgical drainage of pus and necrotic tissue
✅ ICU monitoring for respiratory distress

This condition demands urgent intervention, as delays can lead to sepsis, mediastinitis, or even death. Watch as I manage a case of Ludwig’s Angina with prompt surgical drainage and airway stabilization.

🦷💀 Surgical Management of TMJ Ankylosis in a 10-Year-Old Patient! 💀🦷

TMJ Ankylosis is a condition where the temporomandibular joint (TMJ) becomes fused, leading to severely restricted mouth opening, facial asymmetry, and difficulty in eating, speaking, and maintaining oral hygiene. It commonly results from trauma, infection, or systemic conditions affecting joint development.

🔹 Key Features of TMJ Ankylosis:
✅ Limited or absent mouth opening
✅ Facial deformity (especially in growing children)
✅ Difficulty in speech & chewing
✅ Possible growth restriction of the mandible

🔹 Surgical Management Plan:
✔️ Preoperative Evaluation: CT scan to assess the extent of fusion
✔️ Gap Arthroplasty: Surgical removal of the ankylosed bone to restore mobility
✔️ Interpositional Material Placement: To prevent re-ankylosis (fat graft, temporalis muscle flap, or alloplastic material)
✔️ Post-op Physiotherapy: Crucial for long-term success and function
