Spine anatomy can sometimes surprise us! This patient had lumbarisation – an extra lumbar-like vertebra that makes counting levels confusing.

Lumbarisation is when the first sacral vertebra (S1) behaves like an extra lumbar vertebra → making it look like the patient has 6 lumbar vertebrae instead of 5.

At first, it looked tricky to confirm the exact level of disc prolapse. Careful counting on MRI + X-ray showed the culprit: PIVD at L5–S1.

The procedure was done at the right level, and the patient walked out with excellent relief from pain 🙌

Every case reminds us: precision in anatomy → precision in intervention → happy patients!

🔹 Diagnosis: PIVD (L5–S1) with lumbarisation
🔹 Procedure: Targeted root block / epidural injection
🔹 Outcome: Good (60%) relief in symptoms

Same drug, two faces 😇👹

In Rheumatology, NSAIDs are lifesavers — easing pain, stiffness & inflammation.
In Nephrology, they’re troublemakers — risking AKI, CKD progression & fluid overload.

👉 Moral: One man’s medicine can be another man’s poison.
Always use NSAIDs wisely — right patient, right dose, right duration.

How is she even walking ?????

60year old female with Multiple level profound PIVD with canal stenosis. She has multiple comorbidities (uncontrolled sugars + hypertension). She has difficulty to walk even 100 metres. She is not willing for surgery even after repeated counselling.
Let’s see how we help her walk pain 🆓.

65 years old female patient with severe right L4 L5 radicular pain due to L4L5 and L5 S1 Intervertebral disc bulge. Patient underwent short and quick procedure with X-ray guidance for precision called epidural steroid MIPSI.

Patient had complain of tingling sensation electric shock like pain over L4 and L5 dermatome: a form of sciatica in common words. So slip disc or INTERVERTEBRAL DISC BULGE.

He was on conservative management following which he underwent L3L4 TFESI (epidural injection). It’s a day care procedure in which the target disc space is identified under fluoroscopy. Then with a thin needle epidural space is entered and drug is inserted.
It has been seen to provide a very good result in pain practice.
