ERCP is the standard procedure done to remove stones from CBD and since radiation cant be used in pregnant patients,we used a pre-procedure EUS to confirm the no. of stones and location and post procedure cholangioscopy to confirm complete clearance of CBD. Biliary cannulation was initially confirmed by aspiration of the golden yellow bile upon of insertion of the sphincterotome

A decent tissue core can even be obtained with regular EUS FNAC 19 G needle . Here we used the new EZ SHOT 3 needle to obtain this desirable core tissue from a pancreatic mass lesion

Hi all , Successfully did EUS choledochoduodenostomy after failed ERCP ( twice) for lower CBD tumor causing obstructive jaundice. He previously had resection and chemo for adenocarcinoma at DJ flexure . FCSEMS placed to achieve biliary draiange.. this is an effective alternative to PTBD with superior patient acceptability.. Did an interesting case of polytrauma , rib and pelvic fracture and suspected severe alcoholic hepatitis..serum bili was 23 raised over last 3 weeks.. on conservative management. . CT and usg showed hepatomegaly at variable time points.. initially ictd was placed for right hemothorax and had recurrent effusion after removal of ictd. .. fluid bili was 22 .. so thought of bilio - pleural fistula .. ercp showed contrast blush and extravasation in the right lobe occurring from quaternary and minor biliary radicles. .. biliary sphincterotomy and cbd Stent placed.. .I am sharing ercp images here ..

We had a wonderful focused meeting on endoscopic ultrasound on July 9 and 10 . The interaction with guest faculty was good. There were 4 lectures over 2 days and 9 cases were demonstrated in the conference. The guest faculty are real experts in endoscopic ultrasound and spent their weekend with us during the weekend. Overall it was a wonderful meeting and we all enjoyed thoroughly
Hope to have many more meetings like this in future.

EUS guided celiac plexus neurolysis done in a 65 year old lady with unresectable pancreatic adenocarcinoma. She was not responding to opioids and gemcitabine based chemotherapy. The accurate position confirmed on real time sonographic imaging under EUS guidance offers an accurate delivery of the agents for neurolysis unlike in other routes/radiologic modalities
