Male patient 68 years old presented with exophytic lesion of RT side of tongue posterior part ,incisional biopsy revealed moderately diff.sq.c.carcinoma.
MRI showing just primary lesion and just reactionary cervical l.node (negative neck).also PET scan showing no regional or distant metastasis.
Patient fatty and short neck marking neck dissection
Difficult.
So partial glossectomy with elective neck dissection from lev.I to lev.IV .

Patient 16 years old presented with zygomatic complex fracture,with limited mouth opening and slight enophthalmos.
Treated with indirect reduction through Gillies approach and direct fixation by orbital mesh for the floor and miniplate for zygomatico frontal fracture.
Postoperative no limitations of mouth opening and no limitations of eye movement,enophthalmos was improved.

Female patient 50 years old complaining of swelling in parotid gland RT side since many years ago.
Hesitated to do surgery due to possibility of facial nerve injury.
On clinical examination mobile mass deeply extended.
On MRI showing dumbbell edges and well defined involving superficial lobe of parotid extend to deep lobe with isthmus neck in between two lobes,giving characteristic of dumbbell 💪 like.
Which is go with pleomorphic adenoma.
Surgery done through simple retro mandibular incision.
Tumour completely excised with extra capsular dissection..with preservation of facial nerve.
تمت ولله الحمد اجراء عملية رفع ورم حميد من الغدة النكافية الجهة اليمنى والورم على شكل دمبلص لانه ممتد من الفص السطحي الى الفص العميق للغدة النكافية ..مع خياطة تجميلية ومع الحفاظ على العصب السابع

Interesting case : female patient presented with mandibular skeletal cl III protrusion and chin deviation to left side,with malocclusion,improper occlusal plane,retroclination of lower anterior teeth and narrow upper jaw.
1-presugical orthodontic treatment: arrangement of teeth with correction the occlusal plane,proclination of lower anterior teeth to give real reversal over jet and expansion of upper teeth jaw.
2- Orthognathic surgery:sagittal split osteotomy with set back and rotation of mandible to right side to correct chin deviation.
3- post surgical orthodontic treatment to get intimate and fine occlusion.

تم ولله الحمد اجراء عملية تقويم جراحي لشابة تعاني من سوء الاطباق وتقدم الفك الاسفل وانحراف الذقن .حيث تم اولا اجراء تقويم الأسنان لتصحيح الاطباق وتهيئة الحالة للعملية الجراحية .
بعدها تم اجراء العملية التقويمية الجراحية لتصحيح وضع الفك الاسفل وذلك بارجاعه الى الخلف مع ميلان لتصحيح انحراف الذقن. وكانت عملية مميزة من خلال التعاون المشترك بين طبيب تقويم الاسنان وجراح الوجه والفكين اثمرت ولله الحمد بنتيجة مقبولة .

Male patient 35 years old presented with firm mobile swelling of lt parotid gland sice 5 years old presented,sonography and MRI reveals benign tumor , well defined,looks like pleomorphic adenoma ,
FNA reveals pleomorphic adenoma.
Extra capsular dissection done ,with total excisionof tumor with preservation of facial nerve.
تم ولله الحمد اجراء عملية رفع ورم حميد من الغدة النكافية الجهة اليسرى مع الحفاظ على العصب الوجهي ،لمريض شاب تردد أمتنع من اجراء العملية لمدة خمس سنوات خوفا من اصابةالعصب الوجهي .
وخرج المريض متحسنا دون اصابة العصب الوجهي

Challenging surgery before 15 years ago and during follow up no any recurrence.
Patient came with huge pleomorphic adenoma of deep lobe of parotid gland with extra oral and intra oral extension,closing the oropharynx and pushing the uvula laterally.and pushing major vessels.
Patient only on liquid foods,cannot swallow normal food.
Surgery done through extr oral neck incision without need to lip and mandible splitting.
Tumor completely excised with preservation of facial nerve.
Immediately patient can eat normal food .

اجراء عملية نوعية قبل خمسة عشر عاما وراجعنا للمتابعة .
حيث تم حينها رفع ورم كبير من الفص العميق للغدة النكافية .كان ضاغطا على البلعوم مع صعوبة البلع والاكل .وتم رفع الورم بالكامل مع الحفاظ على العصب السابع ودون الحاجة الى فصل الشفة والفك .

26 years old female presented with endophytic ulcer of RT side of tongue ,associated with fix lymph node of submandibular region,c.t scan and sonography showing central necrosis of lymph node.

Hemiglossectomy done with functional neck dissection,to be later on referring the patient to oncologist for postoperative chemotherapy and radiotherapy.

Sagittal split osteotomy with set back of mandible with rotation to right to correct deviation and occlusion to get cl l relationship.

42 male patient presented with malignant parotid tumor,referred to me from oncologist,with history of incisional biopsy by general surgeon result wa poorly differentiated malignant salivary gland (ductal carcinoma). And befor 4 months ago and because the aggressiveness and size of tumor,oncologist give him 4 cycles of chemotherapy before surgery to shrink the size of tumor .
Total parotidectomy with anterograde approach to preserve the facial nerve..and selective neck dissection RT side .

Young female 26 years old presented with sclerotic mass of upper jaw LT side involving alveolar bone and maxillary sinus extend to medeal wall and floor of orbit ,incisional biopsy revealed osteogenic sarcoma.
Which is aggressive malignant tumor and blood born behavior and metastasis.that why ct scan and pet scan shows no distant metastasis.
Total maxillectomy through Weber Ferguson approach and removal of whole tumor in box ,and reconstruction done for orbital floor with titanium mesh. To be later on after healing referral to oncologist for post operative chemotherapy and radiotherapy.

تم ولله الحمد اجراء عملية معقدة لرفع ورم خبيث من الفك الاعلى بالكامل لفتاة شابة ٢٦ سنة ،،اجريت العملية بالسرعة اللازمة اتجنب انتشار الورم كونه ورم عظمي خبيث وقابلية الانتشار بالدم الى اعضاء اخرى بالجسم .
وتم تعويض محجر العين بشبكة معدنية للحفاظ على وضع العين وعدم هطولها .وتم وضع قالب صناعي سدادة جراحية .وتم خياطة الجرح وخرجت المريضة متحسنة،،نتنى لها الشفاء العاجل ان شاء الله .

25 years old make with motorcycle accident,causing panfacial trauma: complex complicated zygomatic fractures of RT side, lefort l with dentoalveolar fractures of maxilla, lower orbital rim fracture of left side ,parasymphesial fracture of mandible RT side and sub sigmoid fracture of ramus Rt side.
Treated with open reduction and direct fixation with miniplate,and orbital mesh reconstruction of RT side and IMF to get correct occlusion .
Surgery done by Dr.Sabah Abdulaziz/consultant of oral and maxillofacial surgery.
تم ولله الحمد اجراء عملية معقدة ولعدة ساعات لمريض مصاب اثر حادث سير موطور ،ادى الى كسور متعدده في الوجه وعظام الوجنة والفكين الاعلى والاسفل وكانت كسور معقدة تهشمية وتم العملية بنجاح وتم تثبيت الكسور ببليتات معدنية وتم تعويض قاع محجر العين الايمن بشبكة معدنية وتم ربط الفكين باسلاك معدنية للحفاظ على أطباق الاسنان،وخرج المريض متحسنا ولله الحمد

Challenging case:
Old woman 74 years old,hx of Hemiglossectomy of rt side with sq.cell carcinoma since 2years ,on follow up presented with large mass of upper neck ,c.t.scan reveals extracapsular extension of lymph node metastasis at level ll ,showing adhesion and fixation to internal j.vein,,and tethering to carotid artery at bifurcation of internal and external carotid.
Classical neck dissection done removing with mass int.j.vein ,accessory nerve and stencleidomastoid muscle.
Ligation if external carotid artery due to adhesion and with very delicate dissecting and preservation of internal carotid artery .

تم ولله الحمد اجراء عملية استئصال ورم خبيث من الرقبة وهي حالة تحدي ومعقدة بسبب التصاق الورم بالأوردة والشرايين الخاصة بالرأس والرقبة وتم الحفاظ على الشريان المغذي للدماغ بصعوبة بالغة وذلك لان ربطه يؤدي الى الشلل النصفي .
