A 37 year male presented with this finding (as seen in picture below) in the left loin region, 4 days after laparotomy and excision of large 20cm liposarcoma in the left upper abdomen. What do you suspect the problem?

This is a plain X ray of abdomen of elderly female 20 days after straight forward LC. What is her problem and its cause?

A 36 years obese male presented for gastric balloon. His BMI was 38. He said that his blood pressure was unstable and taking Valsartan plus not regularly. His ECG is normal. Liver virology is negative. Medsill gastric balloon is placed under propoful anaesthesia. Devomit 8 mg was given perioperatively. He received Omeprazol ampule just at end of procedure. Postprocedure was put on iv fluids, 2 hours later was good and dicharged home. Another Devomit ampule 8mg was given. About 2 hours later he developed profound shock, BP 70 systolic. Returned to hospital where he found to be conscious but dizzy, thready rapid pulse, BP 80/50.sweaty with cold extremities.
What is the problem?

Interesting case
A 13 years girl presented with a history of sudden attack of lower abdominal pain started at noontime yesterday, followed by vomiting 3 times. In casualty was given iv fluids and medication???. At evening the pain almost gone and her parents returned her to home. Next day, at after noon US was done to her and soon presented to me in my clinic. She was generally ok, having no pain, temp. 37.5. Abdomen was moving with respiration, soft to palpation, mild tenderness in suprapubic region. No tenderness over MacBurney's point. +ve BS.
The US reported the presence of right pelvic collection with features of perforated appendicitis.
What is your plan of management?
