Steroid-refractory Interstitial Lung Disease in Patients Receiving Immune Checkpoint Inhibitors: A Systematic Review and Meta-analysis

This systematic review and meta-analysis evaluates the prevalence, risk factors, and outcomes of steroid-refractory checkpoint inhibitor–induced pneumonitis (CIP). Analyzing data from 2,359 patients across 19 studies, the authors report a pooled prevalence of 12%, rising to 24% among lung cancer patients, with a concerning mortality rate of 32%. Higher risk was observed in males and smokers. The findings emphasize the urgent need for early recognition, close monitoring, and individualized treatment strategies to improve outcomes in this high-risk population.

A board-certified anesthesiologist and Vice Chair of the Clinical Governance Board at US Anesthesia Partners – Gulf Coast, Dr. Zaafran also serves on the FDA’s Analgesia & Anesthesia Advisory Committee and has contributed to the U.S. Department of Health and Human Services Pain Management Task Force.

Effect of Isthmus-preserving Thyroid Lobectomy on Postoperative L-thyroxine Requirement

This retrospective study investigates whether preserving the thyroid isthmus during lobectomy can reduce the need for postoperative L-thyroxine supplementation. Among 125 patients, those who underwent isthmus-preserving lobectomy showed significantly lower rates of postoperative hypothyroidism and reduced reliance on lifelong hormone replacement. The findings highlight isthmus preservation as a key surgical factor in maintaining thyroid function, particularly in patients without thyroiditis, offering valuable insights for optimizing surgical decision-making and improving patient outcomes.
