1. Your Cooperation in adhering to the polices will greatly appreciated. Supporting the Military Working Dog (MWD) mission is the primary purpose of the veterinary facilities of Misawa AB. In the event of a MWD emergency, appointments may be delayed or cancelled as necessary. Use of the Veterinary Treatment Facility (VTF) by privately owned animals is a privilege. In accordance with AR 40-905 (AFI 48-131), veterinary privileges are provided for those individuals enrolled in DEERS and/or have valid SOFA status. Only the sponsor or spouse may register the pet. All SOFA status members pet(s) must be registered with Misawa VTF (35FW136-2901) whether you live on or off base. Registration is comprised of (1) maintaining current records and (2) having proper identification by a microchip. All new pets must be registered at the VTF within 72 hours of acquisition. Please refer to Misawa AB (35 W136-2901) on Animal Care and Control for general base policies in reference to all animals. In order to purchase prescription products from VTF, your pet must have been seen by one of our Veterinarians within the past 12 months. If the prescription is for a serious medical condition, your pet must have been seen by our Veterinarian for that condition within the past 12 months. Prescription refill requests must be submitted 24 hours in advance to allow the veterinarian time to review and refill the request. For safety reasons, it is strongly recommended that parents do not bring children under the age of 12 into the VTF. If children are brought along, they must be accompanied by a parent at all times. If a child is unruly in the VTF and safety is a concern, the pet’s appointment may have to be rescheduled. In accordance with AR 40-905 (AFI 48-131), “Veterinary services that are provided in support of commercial operations may have to be rescheduled. Our appointment availability is limited. When a client makes an appointment and does not keep the appointment, or fails to cancel at least 24 hours in advance, this appointment is considered a no show. This impacts our ability to serve all of our clients. If you are more than 10 minutes late for your scheduled appointment, you will be considered a walk-in and will be seen as our schedule allows. After the second no show, a letter will be sent to your command. If it continues, we reserve the right to suspend your services for 6 months. In order to maintain pets travel readiness, owners should keep a copy of the annual Rabies Vaccination Certificate on hand. It is the owner’s responsibility to ensure all vaccinations are given in a timely manner. Proof of vaccinations and microchip obtained at an off-base clinic must be brought to the Misawa VTF for current registration. If at any time your pet is lost or passes away, you must notify the VTF as soon as possible so that we may update your pet(s) record. If your pet passes away, you will be required to furnish proper cremation/disposal/euthanasia receipts before the chart will be inactivated. If you do not recover your lost animal before PCSing, a letter from your chain of Command is required in order for you to out process. All pets must be leashed or kenneled while in the VTF. Pets with a history of aggression may be required to be muzzled during the visit. During appointments, all privately owned animals will be restrained by veterinary clinic personnel with the minimum effective amount of restraint. An animal that cannot be safely restrained may be refused services, or be required to be sedated in order to be seen.
2. Only the registered owner or spouse is authorized to make decisions regarding care of the pet. If at any time it is necessary for someone other than the owner to provide care, we highly encourage clients to obtain a power of attorney (POA) for this pet to be treated. In the absence of a power of attorney, written approval can be obtained by the owner of the pet by filling out a pet sitter authorization form.
3. Convenience euthanasia will NOT be performed at the Misawa VTF. Euthanasia will only be conducted when a valid need is apparent and diagnosed by the VTF veterinarian.
4. Pet abandonment is a crime punishable under the UCMJ. If you re-home your pet, you and the new owner must complete transfer of ownership paperwork within 24 hours of the transfer (35FW136-2901).
5. We do not have 24/7 emergency services. Please develop your own emergency plan for your pet(s) with an off base veterinarian. A list of the local veterinarians is available at our facility in the black box in between the doors or on our page.
6. Payment is due in full at the time of service.
7. If a client becomes disruptive or abusive with our staff, clients within our facility, or with any animal in our care, we reserve the right to suspend all services indefinitely. One written warning will be provided for all initial infractions. Subsequently you and your command will be notified in writing about suspension of services.

‼️‼️ Updated information: The FAVN is only a requirement to enter Japan. It is not requirement to travel back CONUS.
‼️‼️ The animal evaluation card should be included in your NEO folder, an example is attached below.

It’s that time of year to get your NEO folders ready in case of an emergency. It’s especially important to ensure your pet(s) are up to date with the Rabies vaccine. If you have your pet(s) vaccines that have been updated from an off-base clinic, please provide us a copy by either coming into the clinic or emailing it to us so that your pet(s) vaccine records are current.

In the event of a natural disaster, act of war, or other scenario requiring a mass evacuation from Japan, we want everyone’s pets to be included in this evacuation. Below are tips for ensuring your pet will be allowed on the evacuation flight if necessary. Remember, if these criteria’s are NOT met, the flight line may REFUSE your animal, and they will be forced to stay behind.

• Rabies Certificate is presented, is in English, and is up-to-date
• NEO packet Health Certificate (DD2209, issued by the Misawa VTF) includes your pet’s current Rabies vaccine

Things that may result in your pet being denied from an evacuation flight

❌NEO/EEP packet Health Certificate (DD2209) has an expired Rabies vaccine on it and is already completed by someone other than a veterinarian.
❌Rabies vaccine certificate is missing, in Japanese, or is not up-to-date
❌Your pet does not have an appropriately sized crate for travel

We understand that many pets have acquired off-base care due to many reasons which is why it is important to update records with Misawa VTF ANY time your pet receives vaccines off-base so that we can provide you new NEO/EEP paperwork, and make sure your pet is squared away for evacuation.
