Community Health Promoters (CHPs) play a critical role in promoting access to health care services, encouraging utilization of available resources, connecting individuals with appropriate services and supporting implementation of health initiatives. During the rollout of the IMARA Package, which CHAK in collaboration with other partners is implementing within the Evaluation of Integrated Services (EIS) study, we met with CHPs in Kitui County to discuss their role in advocating for integration of HIV/AIDS services at community level.
The CHPs assured the study team that they were already integrating HIV/AIDS and NCD services during their household visits and would continue to improve delivery with the training and mentorship offered by the study.
There is increased impetus at national and subnational levels for HIV services to be integrated to routine primary care. However, there is limited evidence to guide decision making on the approach, feasibility, effectiveness, and cost-efficiency of such integration. It is for this reason that CHAK (lead partner), UON, NASCOP, D-NCD, Kitui, Kiambu and Nairobi counties are undertaking the EIS study which aims to evaluate the feasibility, systemic requirements, cost implications, and acceptability of integrating HIV and NCDs care in selected health facilities, and implement an integration intervention package to respond to evaluation findings in the three counties. It was an exciting day as we rolled out the IMARA package of care at Kyuso Level 4 Hospital in Kitui County. The IMARA Package, being implemented within the Evaluation of Integrated Services pilot study, is a locally driven, evidence-based framework for delivering integrated HIV and non-communicable disease (NCD) care in primary health care settings.
IMARA is an acronym for Integrated Model for Accessible, Resilient and Accountable Care.
CHAK and project partners, UON, Kitui County and NASCOP held discussions with the Kitui CHMT and Kyuso Level 4 Hospital management team to identify opportunities and modalities for integration of NCD and HIV/AIDS services.
The care package shall: I – Institutionalize Standards and Tools; M – Mobilize Capacity and Systems; A – Align Service Delivery; R – Review Performance and Quality; and, A – Anchor Sustainability and Community Ownership
The roll out was also attended by representatives from the study Intervention and Control sites in Kitui County.
This pilot study aims to evaluate the feasibility, systemic requirements, cost implications, and acceptability of integrating HIV and NCDs care in selected health facilities and implement an integration intervention package to respond to evaluation findings in three counties in Kenya - Kitui, Kiambu and Nairobi.
The study is being carried out in 50 sites in the three counties and will provide a solid basis for integration of health services. Department of Health Services - Kiambu County Kitui County Ministry of Health & Sanitation CHAK Guest House & Conference Centre Chak Cbsl Wekesa Barasa

CHAK has signed an MOU with renewable energy developer Yieldplay Energy Pty Ltd towards installing cutting-edge renewable energy systems and advanced technologies in the CHAK member network, thus ensuring better medical outcomes and stronger healthcare systems.
The strategic partnership directly addresses energy challenges in the health sector, bringing clean, reliable, and affordable power to CHAK member facilities and ensuring better care for the communities they serve.
The initiative will be rolled out in phases, with the first phase targeting 150 healthcare facilities across Kenya. These sites will be equipped with renewable energy solutions to ensure uninterrupted service delivery, particularly for critical medical operations, laboratory services, and cold chain management for vaccines and medicines.
The benefits of this groundbreaking initiative include significant cost savings on energy, energy security, job creation and economic boost, environmental responsibility and capacity building and skills transfer for local technicians to ensure long-term operational efficiency and expertise. The journey to self reliance in provision of HIV/AIDS services is well underway. Integration of services is no longer just a buzz word but is taking place at all levels of the health care system. Today, health facilities supported by the CHAK-CHAP Stawisha HIV/AIDS Programme funded by PEPFAR/CDC had a robust discussion on sustainability of services in the face of major shifts in donor funding. Key among the actions towards sustainability is empowering patients to own their care, including enrolment in SHA. Our discussions also centred on Human Resources for Health, self care and mental health in the face of funding cuts. Participants were keen to discuss and learn from each other on all aspects of integration including dispensing, laboratory, among others.

A major set back for HIV/AIDS care in Kenya is the lack of a SHA package for PLHIV to enable them seek care at higher level health facilities.

CHAK and partner faith-based organisations have taken advocacy efforts to the highest levels, both locally and internationally, to ensure continued support for provision of health care services for the poor and vulnerable populations served by their networks. However, it is clear funding for HIV/AIDS care and treatment will not go on forever and the journey to self reliance must progress. In the words of one of the participants, the only permanent thing is change...and of course God.
