SEVERE APLASTIC ANAEMIA | Stem Cell Transplant Offers Renewed Hope

A new study sheds light on how stem cell transplantation (HSCT) is transforming outcomes for individuals living with severe aplastic anaemia (SAA) — a rare, life-threatening bone marrow disorder that can progress to more serious conditions such as myelodysplastic syndrome (MDS) or acute leukaemia.

Once considered too risky or suitable only for younger patients, HSCT is now safer and more accessible than ever, thanks to advances in donor matching, conditioning regimens, and post-transplant care. Zieve’s Syndrome

Definition:
A triad of clinical findings seen in patients with alcoholic liver disease:
1. Hemolytic anemia
2. Jaundice (not solely due to hepatic failure, but also hemolysis)
3. Transient hyperlipidemia

Pathophysiology:
• Exact mechanism not fully clear.
• Believed to result from:
• Alcohol-induced liver injury → impaired lipid metabolism.
• Abnormal lipoproteins destabilize RBC membranes → hemolysis.
• Alcohol and acetaldehyde directly toxic to RBC membranes.

Clinical Features:
• Occurs in chronic alcoholics with acute alcoholic hepatitis or withdrawal.
• Symptoms: jaundice, fatigue, abdominal pain, nausea, vomiting.
• Lab findings:
• Hemolytic anemia (↑ reticulocyte count, indirect bilirubin, LDH; ↓ haptoglobin).
• Hyperlipidemia (transient, can normalize within days to weeks).
• Abnormal liver function tests (AST > ALT, ↑ GGT).

Management:
• Alcohol cessation (mainstay).
• Supportive care: fluids, nutrition, transfusion if severe anemia.
• Lipid abnormalities usually resolve spontaneously once alcohol is stopped.
• Important to differentiate from other causes of jaundice and anemia in alcoholics (e.g., GI bleeding, cirrhosis, hepatitis)
