We are bursting with pride to announce the incredible achievement of our esteemed doctor, Dr. Dharmaraj Karthikesan, a renowned Consultant Interventional Cardiologist! He has just graduated from the prestigious University of Oxford with an MSc in Clinical Trials! 🎉

This is not just a personal triumph, but a landmark moment for Malaysia! Dr. Dharmaraj Karthikesan is the first Malaysian to obtain an MSc in Clinical Trials from the University of Oxford! His dedication and pursuit of excellence are truly inspiring.

Adding to his remarkable list of firsts, Dr. Dharmaraj is also the first Malaysian to receive an ESC (European Society of Cardiology) grant, further solidifying his position as a pioneer in cardiovascular research and development in our nation.

Kami amat berbangga untuk mengumumkan pencapaian luar biasa doktor kami yang dihormati, Dr. Dharmaraj Karthikesan, seorang Pakar Perunding Kardiologi Intervensi. Beliau baru sahaja menamatkan pengajian dari University of Oxford yang berprestij dengan ijazah MSc dalam Percubaan Klinikal! 🎉

Ini bukan sahaja kejayaan peribadi, tetapi juga detik bersejarah bagi Malaysia! Dr. Dharmaraj Karthikesan adalah rakyat Malaysia pertama yang memperoleh MSc dalam Percubaan Klinikal dari University of Oxford! Dedikasi dan usaha beliau mengejar kecemerlangan sememangnya sangat menginspirasikan.

Our second TAVI workshop, being proctored by Dr Gurbhej Singh who came all the way from India to assist our TAVI team in Hospital Sultanah Bahiyah, lead by Saravanan Krishinan , our head of department, Kantha Rao , consultant cardiologist and Dr Tan Chai Yih, fellow cardiologist, We successfully implanted 2 MyVal in 2 extremely high risk aortic stenosis patients who was deemed too frail for open heart surgery. The first case was straight forward implantation with no major complications. The 2nd case was a bit more challenging, from calcified femoral access which required peripheral intra-vascular lithotripsy prior to sending up our devices, and also a Chimney left main stenting post device implantation. To top it , femoral access was too calcified to be secured by vascular closure device and we had our CTC standby Mr Sotheenathan to perform an arterial cut down to secure the bleeding. Both patients were well and planned to be discharged in a day or 2 time. This is a milestone for the department and also a glimpse of hope for the people of Kedah whereby geriatric patients can be offered such procedure whenever needed and indicated.

Congratulations to Dr Nancy Virginia and Dr Kang Huan Yean who presented our single centre data at TCT (Trancatheter Cardiovascular Therapeutics) 2024 held at Washington DC United States on the usage of drug coated balloons (DCB) in chronic total occlusion (CTO ) and hybrid approach in unprotected left main intervention mentored by Dr Dharmaraj Karthikesan. This approach in intervention may play important role in the future by leaving nothing behind ( metal free implantation).

A great and historic moment for both Cardiology Department of Hospital Sultanah Bahiyah, Alor Star and Hospital Pulau Pinang, successfully collaborated in implanting the latest Aurora Extravascular Implantable Cardioverter Defibrillator (EV ICD) on 4th of July 2024.
The procedure was safely and successfully performed under general anaesthesia by Dr Kantha Rao Narasamuloo (Senior Consultant Cardiologist and Electrophysiologist) together with Dr Saravanan Krishinan (Head of Department & Senior Consultant Electrophysiologist) from Hospital Sultanah Bahiyah Alor Setar under live proctorship from pioneer of EV ICD, Dr Ian Crozier ( Senior Consultant Cardiologist and Electrophysiologist from Chrischurch Hospital New Zealand ).

This EV ICD implantation was the 1st in a public hospital in Malaysia and 2nd in South East Asia, making this event even more memorable. The event was also graced by the presence of Head of Department & Senior Consultant Cardiologist Dato’ Dr Mohamed Jahangir Abdul Wahab during the implantation. A great teamwork with strong support from cardiothoracic team headed by Mr Sotheenathan Krishinan (Senior Consultant Cardiothoracic Surgeon) and anaesthesia team headed by Dr Ang (Senior Consultant Anaesthetist).

This procedure was made possible and tailored for a 50-year-old gentlemen with end-stage renal disease on regular dialysis for 8 years via left brachiocephalic fistula with pre-existing hypertension, dyslipidemia and ischemic dilated cardiomyopathy.

He sustained life threatening ventricular arrhythmia to Hospital Pulau Pinang requiring inpatient implantation of implantable cardiac defibrillator device. The patient also had pre-existing peripheral arterial disease with severely poor vascular access. Despite revascularisation of his coronaries, his cardiac function remained severely poor at 17% which was confirmed by perfusion scan as well. This made him the perfect candidate for an Aurora Extravascular Implantable Cardioverter Defibrillator (EV ICD) in view of his background health history that wasn’t favorable for a transvenous ICD lead.

With the support of Medtronic team Malaysia and United States of America, the HEART team from Hospital Sultanah Bahiyah, Alor Star and Hospital Pulau Pinang were able to carry out this breakthrough event. The patient remained well throughout and post procedure and is expected to be discharged in 1-2 days time. The device had been tested and is capable to fully carried out it’s role of preventing the patient from a fatal arrythmia.

The patient and immediate family members were grateful to all parties involved for providing such up to date and advance treatment in the field of cardiology.

A new Era in catheter ablation of atrial fibrillation
- Pulse Field Ablation (PFA).

Department of Cardiology HSB is proud to be one of the first few heart centres in Malaysia to use this revolutionary ablation technology and performed our very first PFA (using FARAPULSE) TODAY(8/5/24) under the proctorship of A/Prof Dr Seow Swee Chong who came all the way from National University Heart Centre, Singapore.

2 patients were successfully treated in today’s “PFA Workshop” headed by our head of department and consultant Cardiac Electrophysiologist Dr Saravanan a/l Krishinan. The two patients were retired public servants,both achieved complete PV isolation using a 31 mm basket catheter, confirmed by 3D EAM ensite mapping and exit block. The second patient had a hybrid procedure of Left atrial appendage closure using LAMBRE device, as unable to tolerate oral anticoagulant.
