Our national birth rate has reduced greatly to less than 2, which means a couple having less than 2 children per family. This will eventually lead to ageing population… Remember this, having 2 children in family can only substitute the 2 parents. Having 3 children or more makes the population grow…

A pre-pregnancy assessment for couples who wishes to conceive is important.
1. Start Folic acid preferably 3 months before pregnancy.
2. Optimize if have medical conditions. Need to consult your physician before embarking on pregnancy. Some conditions are contraindicated to get pregnant. Eg. Optimize sugar level for established diabetic patients, or for SLE patients, must be in remission for 6months. Medicine to be changed to those safe in pregnancy. Heart disease pt with Eisenmenger syndrome not advisable to get pregnant.
3. Infective screening, treat if necessary.
4. Ultrasound to see if any uterine or ovarian growth , location of the growth, the size etc.
5. Genetic counseling in couples with inherited genetic diseases eg Thalassemia is common in Asian countries.

She is so pretty and smart, another living proof of pregnancy conceived via using ovulation induction medicine. Ovulation induction agents is useful in some patients whom has ovulatory dysfunction, which accounts for 25% of infertility cases. Come forward for assessment, sometimes a little help can make your dreams to parenthood come true..❤️

Incidence of infertility has increased over the years. Be open to assessment, sometimes a little help can transform your life….infertility treatment can vary from just fertility counseling, using ovulation induction agents to Intra-Uterine Insemination (IUI) or In Vitro Fertilization (IVF).

Don’t be afraid, take the 1st step forward…come for assessment…

Historically, we mostly investigate the female partners for the cause of recurrent miscarriages. Of recent years, it is believed that high s***m DNA fragmentation could be a reason for recurrent miscarriages. Unexplained infertility can sometimes be explained from poor egg quality under the microscope during IVF and from high percentage of s***m DNA fragmentations..

High percentage of s***m DNA fragmentation has correlation with poor embryo development, poor embryo implantation and higher miscarriage rate. Therefore, it is encouraged to do S***m DNA fragmentation test.

Recently I had a chat with a friend whom is also a parent of a medical student… this is what I shared with the parent..because this is what I believe..

The best teacher to us as medical students or those in medicine profession is our patients… There is no 1 patient who has the same presentation of an illness. All our bodies are different and we react differently to the illness and to treatment..

Getting the diagnosis or problems list is important because it helps us the focus on how to help/treat the patients… and 80% of the diagnosis comes from history taking, the other 20% from clinical signs and investigations.

The patients may come with a group of symptoms…they don’t have a label on their forehead- this is my diagnosis. So, we need to talk to the patients, understand the symptoms, the signs…and tie everything together..to get to the diagnosis. This reminds me of my primary school days, when I love reading Nancy Drew and Hardy boys series of solving mysteries… to me, practicing medicine is efficiently solving the patients’ health problem and return them to their loved ones. Hospital is not their home…

And now that I am in Reproductive Medicine, the task is to solve all those TTC with their reproductive issues… this is my passion now…

And also not forgetting….it is important to have humility, empathy, compassion, love and kindness n good communication with patients, apart from practicing good medicine…

Good medicine without the human touch , is not enough… ❤️❤️❤️
