Some important revision points from this article.

✅️ Nebulisation for asthma is driven by oxygen, and nebulisation for COPD is driven by air to avoid worsening hypercapnia via overoxygenation. Transition to inhaler should happen ASAP.

✅️ Prednisolone 1mg/kg/day (up to 50kg) for 5-7 days (Asthma), prednisolone 40mg/day for 5 days (COPD). Steroid with primarily glucocorticoid effect is preferred to those with mineralocorticosteroid effect.

✅️ Antibiotics can be used in critically ill AEBA with concern of concurrent lung infection, and should be used in critically ill AECOPD who are on NIV/MV, or those with increased dyspnoea/purulent sputum/increased sputum volume.

✅️ IV Magnesium 2gm over 20min can be used in asthma/COPD- may enhance the bronchodilator effect of SABA.

✅️ Methylxanthines and terbutaline are NOT indicated in both AEBA and AECOPD.

✅️ Data on HFNC in severe AEBA are very limited. NIV should be considered before HFNC for AECOPD with respiratory acidosis, but HFNC can be an alternative if patient does not tolerate/would benefit from alternating between these methods.
