I was having a Tete a Tete with my colleague who is a medical doctor while we were discussing pathology as regards cytogenetics and molecular biology. Though our center of discussion was on mutations and how mutations results to different kind of disorders which some are congenital. Then, we entered genetics with regards to cancer.

Cancer is a growing menace we face in our society these days and many have died while many are in various hospitals spending scarce financial resources to get treatment. In fact, many have resorted to spiritual houses as they can no longer keep up with orthodox therapeutics.

While we were discussing, we actually landed in something very important I will like to share with us as simple as possible so we get to understand.

There is a particular gene called p53 gene and nicknamed the protector gene because it acts so much in regulation of cell growth and programmed cell death of possible cells with a damaged gene. This gene is located in something known as a chromosome and this said chromosome usually has 2 arms. Studies show that an arm of this chromosome is defective in some individuals. Though the other arm can compensate. But when the other arm is damaged by external factors like irradiation, poor dietary lifestyle and the likes, p53 is therefore defective and may not perform it's role and cancer sets in.

This is the more reason we need to be very much keen on lifestyle adjustment and readjustment. Leave that smoking, alcohol, excessive sugar and junks alone. You will not die if you don't take soda at all. Do away with it. You don't know the category of individual you are. Nevertheless, don't take chances and 👇

Dear ladies.
That persistent UTI you have may not be about the ineffectiveness of the drugs you use but how you wash your iskaba, iskelebete, iskeloboto 😂 sorry how you wash your a**s after defecation.

It is very wrong to wash from the a**s to the vulvova**nal opening (Note the opening that is very visible to you is not your va**na but v***a). This way you can transport the bacteria inhabiting your guts and drop them into your your va**na which has your urethral opening connected to it. This can in turn cause a urinary tract infection (UTI).

NEXT TIME! For those of u using tissue paper, wipe from front to back. This means wipe your vulvova**nal opening down to you a**s and for those using water, wash from front to back. If by mistake you wash the a**s first, thoroughly wash hands with detergent before taking your hands to your v***a. Note, water is more hygienic in washing our a**s after defecation.

So at times it's not that those drugs don't work, neither is it your toilet that's causing you infection but you keep reintroducing those stuffs where they don't belong.

Scientist discover therapy for patients with severe autoimmune disease.

Five people with severe autoimmune disease have become the first in the world to receive a groundbreaking therapy that uses genetically altered cells to drive the illness into remission.

The four women and one man, aged 18 to 24, received transfusions of modified immune cells to treat severe lupus, an autoimmune disease that can cause life-threatening damage to the heart, lungs, brain and kidneys.

The treatment drove the disease into remission in all five patients, who have now been off lupus medication for between three and 17 months. Doctors say the apparent success raises hopes for tackling other autoimmune conditions such a rheumatoid arthritis and multiple sclerosis.

Lupus, or systemic lupus erythematosus, develops when the immune system mistakenly attacks healthy tissues and organs. The causes are not well understood, but researchers believe it may be triggered by viral infections, particular medicines, and changes in the body around puberty and the menopause.

The condition affects about one in 1,000 people, and far more women than men. It is hard to diagnose because the symptoms often flare up and settle down, and overlap with those of several other diseases. While mild in many people, lupus can cause extreme tiredness, organ damage and pain in the joints and muscles. One of the most common signs is a distinctive skin rash over the nose and cheeks.

Doctors in Germany treated five severely ill patients with CAR T-cell therapy after other treatments failed to improve their symptoms. The approach has proved successful at combating certain cancers since it was first used in a leukaemia patient in 2015. CAR T-cell therapy involves collecting the patient’s T-cells – a key component of the immune system – and modifying them so that they attack new targets, such as cancer cells, when infused back into the body.

In the latest work, doctors took T-cells from the lupus patients and modified them so that, on re-infusion, they attacked the patients’ B cells. In lupus, B cells churn out autoantibodies, which instead of defending the body against invading pathogens, attack healthy tissues instead.

According to the study in Nature Medicine, the therapy in effect wiped out the patients’ aberrant B cells and dramatically improved their condition. The disease affected multiple organs in all five patients, but after the therapy severe symptoms including arthritis, fatigue, fibrosis of the heart valves, and lung inflammation all cleared up.

Blood tests on the patients showed that their B cells recovered about four months after the treatment, but they no longer produced aberrant antibodies and the patients remained disease-free. Writing in the journal, the authors speculate that the therapy led to a “rebooting of the immune system”.

“We are very excited about these results,” said Prof Georg Schett, a rheumatologist who led the work at Friedrich-Alexander University in Erlangen-Nuremberg. “Several other autoimmune diseases which are dependent on B cells and show autoantibodies may respond to this treatment. These include rheumatoid arthritis, myositis and systemic sclerosis. But also diseases like multiple sclerosis may be very responsive to CAR T-cell treatment.”

Schett’s team was keen to ensure the therapy did not impair the patients’ immune systems and leave them at greater risk of infection. To test this, they assessed the patients’ responses to multiple vaccines, including measles, rubella, mumps, hepatitis B, teta**s and diphtheria, before and after the therapy. The patients’ immune responses were not substantially different after the treatment, suggesting that it primarily targeted the wayward autoantibody-making cells.

“This is an excellent study which promises to extend the scope of CAR T-cell therapy, which has thus far seen its major impact in the treatment of blood cancers, to autoimmune diseases like lupus which are in some patients poorly controlled with other medicines,” said Dr Rahul Roychoudhuri, who studies immune system regulation in inflammation and cancer at the University of Cambridge. “I am very excited at the prospects for this form of living therapy in indications beyond cancer.”

I decided to do a review of the patients I attended to for the past 1 month of my new placement.
I decided more on focusing on High Vaginal swab and see if my result will actually get to give me the required information on the disturbance of our normal microbiome. This is because of all the patients I attended to that has to do with bacteria culture ranging from stool culture, blood culture, sputum culture and urine culture, HVS actually placed itself as a perfect case study because.
1. It's not a sterile anatomical site
2. The frequency of patient coming for HVS outweighs all other with urine culture coming second and blood culture taking the third spot.
Though, blood culture being a unisex case study, it's limitation is that it is a sterile sight and does not fit in my main purpose for the review.
In all, I attended to 20 female patients and of all 3 which is 15% had Candida spp. as isolated organism, 4 which is 20% had Gardnerella spp. as isolated organism and 1 which is 5% had Neisseria gonorrheae as isolated pathogen.
The rest were either normal va**nal flora or no organism were isolated at all.

Note other factors most especially that of Gardnerella spp which includes amount of puss cells and clue cells were put into consideration before pathogenicity was concluded.

This brought me to the conclusion that women are still not well informed about the importance of of the microbiome in their va**na and also shows that many still engage in acts that affect the serenity of their va**na thereby brewing the possibility of microbial radicalism. Hence, the reason for today's lesson.

All patients in this review are immunocompetent. This ruled out the possibility of opportunism as a result of immunosuppression.

This brought me to the conclusion that women are still not well informed about the importance of of the microbiome in their va**na and also shows that many still engage in acts that affect the serenity of their va**na thereby brewing the possibility of microbial radicalism. Hence, the reason for today's lesson.

Dear women, your va**na is an abode for many beneficial microorganisms that help to maintain the balance therein. Notable of these organisms, is the one called lactobacillus. This organism have so many benefits and one which is most important in the acidity of the va**nal wall. This helps to keep other organisms that may go on rampage at Bay like the Cadidas and Gardnerellas.

When you engage in unhealthy va**nal cleansing, or application of chemicals in the name of va**nal tightening and whichever reason you seem it fit, you are indirectly harming these organisms thereby increasing the chances of other organisms that were initially kept at Bay to go rogue.

Note:
1. Do not insert things into the va**na
2. Avoid steaming the va**na in the name of cleaning. This will heat it up and distorts it temperature thereby affecting the normal Flora
3. Avoid douching
4. Avoid the application of chemicals in the name of va**nal tightening.
5. Give your va**na adequate aeration i.e. allow fresh air to pe*****te that region. Don't always go to bed with tight un**es.
6. Avoid the indiscriminate use of antibiotics and birth control pills (contraceptives).

Our lesson for today.
1. Whenever you visit the hospital, avoid indiscriminate touching of surfaces.
2. Whenever a doctor, phlebotomist, or nurse attends to your patients wether during ward round, or phlebotomy procedure, ensure that the same gloves used on patient A is not used on your patient most especially when the procedure is invasive.

This is because, you or your patients can get infected with nosocomial infections otherwise known as hospital acquired infections. Studies have shown that hospital acquired infections contribute largely to the global health burden if multi-drug resistant (MDR) organisms.

BE VIGILANT. join the fight against MDR Organisms.
