The rain has a mind of its own, you definitely don't decide what time it pours. This morning it has decided to pour beautifully. I am more concerned about those children going to school. I noticed the lack of preparation and intentionality in how they were dressed for school. I believe the idea is to rush them to a bus stop so they can quickly get to school before the rain gathers momentum again. The challenge with this weather is the effect it can have on the immune system. Prolonged exposure to rain may lower the body temperature thus predisposing the child to viruses that survive better equally in cold weather.
Children who have issues with airway allergies are susceptible to various flares when exposed to the rain. Damp weather exacerbates spore growth in fungi, leading to these and other allergy problems in susceptible individuals.
The second challenge is the footwear. Due to the rain, they may wear rubber shoes that won't be damaged by the rain. However as the shoes wear the friction is reduced and the possibilities of a fall is increased.
So proper dressing, umbrellas, delayed resumption time on those days should be explored.

Nothing wey Musa no dey see for gate o; nothing wey doctor no dey see for consultation.

That is how one I will call Alhaji Mustapha entered the consulting room this day and his request was simple: "I want you to take over the management of my wife's pregnancy", he said.

The lady in question, Chioma, was 20 years old and she couldn't communicate at all in English. I thanked Alhaji for the trust and motioned for Chioma to climb the examination couch. Everything was going to be done through an interpreter and sign languages. Alhaji understood some Yoruba and a bit of pidgin. He came with some women who also helped in translating everything I needed to Chioma. All in a day's job, I told myself.

Upon examination, I quickly discovered that Chioma was already in the latent phase of labour (there are 2 major phases of Labour: latent and active).

I asked for the last menstrual period and the EDD but no one there had an answer. However, they came with some paperwork that amazed me, considering that the couple didn't appear urbanized. Chioma had done about 3 ultrasound scans, several urine tests, and several blood checks. They also produced an xray that measured the size of her pelvis; all from where she was coming from (it wasn't a hospital though).

I asked Alhaji why he wasn't comfortable continuing there; he replied that he didn't believe that his wife's first pregnancy should be attended by a "midwife" without a doctor's supervision or access. I was truly impressed.

There are so many I have seen who have had access to western education and urban city living; yet, they would commit first time mothers to quacks or untrained birth attendants.

Pregnancy should always be attended to by a skilled midwife or obstetrics practitioner. Nigeria's maternal mortality rate is still among the highest in the world, with an estimated 512 deaths per 100,000 live births; which is nowhere near the SDGs target of 70 per 100,000 live births.”

There are a lot of myths surrounding sickle cell anemia, some of them are products of fear and others borders on outright fables. I will be journeying throughout this week creating awareness about SCD, the SCD warriors, the things to watch out for, how to understand what they go through.

Consultation Room Chronicles (96)

If you have been around me well enough, you will know I love a particular snack; so much that if I see a child with it, I can ask for some without shame. My love for this brand of chin-chin is that deep. I know, it doesn't even make sense!

So you can believe me when I say that I understand how people can have an almost cult like obsession for certain foods, places or entities. Arsenal fans, you all can understand clearly what I am driving at, right? That stray bullet was not intended.

This is why it is so frustrating when I have to deal with patients who are attached to a view point or activity that is detrimental to their healthy living; and most of the time, those views are no more than mere myths that have been passed around or handed down. Such as myths about the route of child delivery, myths about the p***s size, myths about breast size, myths about IV Fluids, myths about blood and blood products, too many more to count.

Let me call our patient today Hilda so you will remember her. She was a known hypertensive who decided on her own to switch her medication to another one that someone told her was more effective, and she did it twice in 3 weeks. She fiercely believed she had 'strong typhoid that has eaten deep into her system', whatever that means. The most troubling of her concerns were the feelings she had in her head and body. She considered her case a spiritual one that required the attention of a prophet. I just sat there in that consultation, in awe of the utter nonsense she was speaking but concerned about how to help her with that mindset she held on to.

There was a sudden twist during the consultation when, without my consent, she called the prophet on the phone who very audaciously proceeded to tell me how to be a doctor. The prophet told me to give her drips to flush the system, explaining how he also has a 'nurse' that helps in this flushing. The prophet said that she just needed a preliminary work up, then the management of her health situation will be finalised at the prophet's place. story continues in the comments section.

One interesting thing about humans is that we have reasons for doing what we do. Companies pay huge amounts of money to consultants and specialists to help them unravel the mystery and psychology of people so that they can know the reasons why people buy and profit from the knowledge of those reasons.

The other day my wife and I were driving towards Ikoyi when we saw a Honda Accord car by the road with the owner standing by it. We could see that the front wheel bearing fell out while the vehicle was in motion. my wife drew my attention to it, reasserting the concerns she expresses whenever I tell her I love Honda cars. Her fears came from our experience with the first car we bought after marriage, which was a Honda Accord.

On our 6th wedding anniversary, I wanted to be that good man and romance my wife, so I got a hotel room with a view of the ocean in the upscale part of VI for us to spend some time together. We were there for an awesome 2 days. It was all good until we were driving home. Right in front of Silverbird Cinemas, along Ahmadu Bello Way, our car broke down just like the one we just passed on the road. That day, we were blessed to have the LASTMA officers around who graciously called us a towing vehicle to get us off to my mechanic before we could go home by public transport.

So you can surely understand why my wife reminds me every time the issue of Honda comes up and why she will definitely discourage anyone from buying a Honda car.

Such is the psychology of people. The source of decision making is often deep seated and emotional and not as logical as we like to think it is.

What I have not been able to unravel is the process that would make a woman remain in the home with man whom she is sure has another "side-hen". That man is not funding or providing for the home; rather, it is the woman funding that project with time and money. All of that is to bring you to see Amanda, and the mind of one who is 'in love' if we can call it love.

No matter what you've been told, children are such darlings; they bring smiles, and they change the dynamics of a family structure; their arrival confers responsibility, and changes the nomenclature for close family members.

Just think about it. You were simply Dele before; the arrival of your nephew has now turned you into something special - somebody's uncle Dele. You may be just Tonia before; the arrival of that your friend's sweet angel elevated your status; you suddenly are now a godmother. For many, they became grandparents. You catch the flow, right?

I must be honest so I have to also mention how the joy they bring is also associated with a lot of emotional and psychosocial issues. You have to mind your temperament in their presence; visits to their homes may also be reconsidered in the event that you are owing them gifts.

The consistent fact that is sustained in all this is that they need to be cared for and protected. This is why a seasoned family physician is not alarmed when a child is sick. Even if the whole town (just kidding!) is at the reception, frantic; especially if the admission has gone beyond the 24 hours span, a good doctor is trained to carefully sieve through the crowd to ask for the direct caregivers/parents. There are those instances where the domestic staff give better and more accurate information than actual parents of the child. Sad, but true.

The challenges of parenting in this contemporary society are overwhelming, you may argue. Okay o. I hear you.

On this day, our beloved Ugochukwu was brought in by his Dad, with very high fever and a bad cough. For a 2 year old, that temperature was not safe. We agreed at consultation to investigate.The problem started when at the first prick of the needle we were not able to secure an IV access. The father became agitated and blatantly refused to allow the doctor try again. All pleas fell on deaf ears as he obstructed the giving of care to the child. I intervened and promised to be as careful as possible but he still wouldn't assent. At that point, he insisted on just going home with oral medications for the child.

I know our generation is becoming woke, I understand a lot of terminologies are coming out to describe the relationships some of our parents had with us. For example I am certain my Dad never said "I love you" to me before he went on to glory. But he sold one of his property in a very prime location in Ilorin to ensure we graduate in the university. There was a time 4 of us were in University of Ilorin.
How do you describe that as unloving? Within the best of his exposure and knowledge HE LOVED ME(US).
So be careful before you join the woke gang that will describe parents in unprintable names.

Its beautiful to get a second opinion on your health condition. It is actually very helpful when the patient is a co-participant in their care in the hospital. It is called patient centred care.
We must however allow the doctor to do their job, listen very well, ask questions in a non intrusive or condescending way and finally be the name you are called in a hospital...PATIENT. There is a reason that name was given.

Dear Friend,

Sexual desires are not sinful in themselves, they only become sinful when they veer outside the boundaries God established for them. 
The enemy has always perverted this good part of our humanity by changing the boundaries. It is one area where the enemy has had great success by convincing millions that their sexual desires should determine the course of their lives. The restraints have been removed and lines of decency blurred so that we now hear of body counts or intimacy without commitment. 

For animals, sexual urges are merely physical needs like hunger or thirst. For humans, sexuality is not just a desire, they lead to emotional bonding, spirituality, self-control, and psychological intimacy and in a rapidly changing world, we need to come together as people of faith to establish and redefine those boundaries.
