FIBROIDS PROBLEMS IN NIGERIA
My topic today is for the general online community, but especially for the Nigerian online community because this is an issue that affects us seriously.
Today I will be writing about fibroids. Uterine fibroids are benign tumors, which grow from the muscle layers of the womb. They are the most common benign tumors in females, often asymptomatic, they cause symptoms in about 25% of white and 50% of black women.
Fibroids are named according to where they are found. There are four types
[1] INTRAMURAL FIBROIDS are found in the walls of the womb and are the most common type of fibroids.
[2] SUBSEROSAL FIBROIDS are found growing outside the walls of the womb and can become very large. They can also grow on stalks [called pedunculate fibroids]
[3] SUBMUCOSAL FIBROIDS are found in the muscle beneath the inner lining of the womb walls
[4] CERVICAL FIBROIDS are found in the walls of the cervix [neck of the wombs].
In rare cases, fibroids can turn cancerous
SYMPTOMS
Symptoms related to the location of the fibroids and their size. Important symptoms include abnormal gynecologic hemorrhage [bleeding], heavy or/and painful periods, abdominal discomfort or bloating, backache, urinary frequency or retention, and in some cases infertility. There may also be pain during in*******se depending on the location of the fibroids.
During pregnancy, they may be the cause of miscarriage, bleeding, premature labor, or interference with the position of the fetus.
DIAGNOSIS
Diagnosis is usually accomplished by bimanual examination, better yet by gynecologic ultrasonography[SCAN], magnetic resonance imaging[MRI], and laparoscopy. Fibroids can also be diagnosed during HSG examination.
TREATMENTS
[1] Surgery [a] hysteroscopic myomectomy [b] laparoscopic myomectomy [c] laparotomic myomectomy.
[2] Uterine artery Embolization [UAE]
[3] Magnetic Resonance Guided Focused Ultrasound [HIFU]
Fibroids are very serious issues in Nigeria where I practiced for over 14years. Fibroids became a real issue in Nigeria after the death of popular MEE Mofe-Damijo in 1996 from post-surgical complications from the removal of fibroids. I was a very young sonographer at that time and I attended to thousands of women and up till this day fibroids are still a big issue.
I have been opportune to have traveled to 4 continents in the world [Africa, Asia, Middle East, and Europe] and worked in Africa and Europe as a specialist sonographer, fibroids still remain an issue though much more in Africa. It is also a big business, good pay for the surgeon and also good money to traditional healers.
I have no problems with traditional healers and alternative medical practitioners, but what I believe is that some of them should give true information to the patients.
I have attended to so many women in Nigeria who have visited some of these alternative medical practitioners and they claimed to be able to give them some concoction that will make these women excrete these fibroids, this is not fair, for years I have followed these women and I must confess non received any so-called cure.
I am not against anyone taking any form of cure she wants whether orthodox or traditional, but safety should come first, NAFDAC must confirm that these concoctions are safe for human consumption.
I also noticed that some of the concoctions given to these women with fibroids/infertility problems are the same ones given to women with general infertility problems. From the numerous scans I did on this issue in Nigeria I noticed that their ovaries produce multiple mature follicles than usual, which shows that these drugs contain some natural hormonal substances that stimulate the ovaries and help infertile women to get pregnant.
If such drugs are given to women with fibroids, these could lead to an increase in the size of the fibroids. While these women are looking to reduce the size of the fibroids, the administered concoction increases the size [Please be careful]. These concoctions have also been known to lead to kidney failure.
I want to advise clearly that any woman with fibroids should not get stressed, if there are no threatening symptoms, be relaxed, don’t go running up and down, and get duped.
If you are an infertility patient with known fibroids, don’t always assume that the fibroids are the problem, do tests to check out your fallopian tubes, ovarian functions, s***m tests, etc [Ask your gynecologist for further information]. If you are having excessive bleeding or the fibroids are very large make sure you see your doctor, surgery is best in this instance because the fibroids can twist, or compress the ureters and give renal problems.
I want to specifically talk to Consultant radiologists in Nigeria, Uterine Artery Embolization[UAE] is a relatively cheap and very effective treatment for fibroids. It only needs an X-ray fluoroscopy unit, which is available in some centers in Nigeria.
I have known women in Nigeria who paid as much as N200000 for the traditional cure of fibroids and they are getting no results.
Uterine Artery Embolization is known to have worked, I have attended to some women in Europe that have done it and have seen the fibroid masses shrink, saving the women the complication of surgery.
I recommend that Nigerian radiologists should come to Europe and learn these skills, the Nigerian women need it, they will be saved from the complication of unnecessary surgeries, and the drinking of strange concoctions from some strange healers.

7 WAYS TO KNOW IF YOUR VAGINAL DISCHARGE IS DANGEROUS AND HOW TO PREVENT IT FROM DESTROYING YOUR FALLOPIAN TUBES

PROBLEMS WITH VAGINAL DISCHARGE

Ladies must pay attention to this advice, especially if you wish to become pregnant someday. Pay attention to any possible va**nal infection you may have. I know this is something we do not want to discuss. If the symptoms are not too severe, we put it down to a possible yeast infection and wait it out, praying it will quickly cure itself. In the process, we may use some over-the-counter remedies. But what we should be doing is seeing a doctor. Why? Infections in the va**nal region can be serious. They can lead to long-term problems such as infertility issues later in life. They can make it difficult to become pregnant and they may cause complications to occur once you do become pregnant. They can even signal that you are infected with a possible STD (s*xually transmitted disease).

A discharge is a fluid released from a hollow space like the va**na. The
va**na is a tube 8-12 cm long. It opens at the lips of the v***a and is closed
at the inner end by the cervix (the opening of the uterus or womb). Wetness in
the va**na is produced by mucus from the cervix and from the va**nal wall
during s*xual arousal. Many different bacteria, yeasts, and parasites
(microscopic forms of life or ‘bugs’) live normally in the va**na without causing
symptoms. The va**na is usually acidic because the normal bacteria produce
lactic acid. Normal bacteria help the va**na protect itself from the kinds of
bacteria that cause disease. Candida (yeast) can live in the va**na in low
numbers without causing any symptoms.

Vaginal discharge [or Discharge from the va**na] refers to secretions from the va**na. Such discharge can vary in:
[A]Consistency (thick, pasty, thin)
[B]Color (clear, cloudy)
[C]Smell (normal, odorless, bad odor)

Having some amount of va**nal discharge is normal, especially if you are of childbearing age. Glands in the cervix produce clear mucus. These secretions may turn white or yellow when exposed to the air. These are normal variations.

The amount of mucus produced by the cervical glands varies throughout the menstrual cycle. This is normal and depends on the amount of estrogen circulating in your body. It is also normal for the walls of the va**na to release some secretions. The amount depends on hormone levels in the body.

Vaginal discharge that suddenly differs in color, odor, or consistency, or significantly increases or decreases in amount, may indicate an underlying problem like an infection.

CAUSES
The following situations can increase the amount of normal va**nal discharge:
[1]Emotional stress
[2]Ovulation (the production and release of an egg from your o***y in the middle of your menstrual cycle)
[3]Pregnancy
[4]Sexual excitement

ABNORMAL VAGINAL DISCHARGE
Unusual mucus or other substances coming from the va**na is a common problem. The discharge is often due to infection and is frequently associated with pain, burning, itching, and painful urination. Not all infections are s*xually transmitted, so don’t assume that va**nal discharge means that you have an STD. There are a number of possible causes:

Inflammation of the va**na. Called vaginitis, is the most common reason for discharges and is usually caused by infection. There are three main types of va**nal infections, all of which can be treated with oral or va**nal medications. Each infection tends to produce a distinct discharge:

[a] Thick, white cottage cheese-like discharge, itching, irritated skin—yeast infection, or candidiasis. Women with diabetes and those taking antibiotics are more likely to develop this type of infection. Most women will have at least one yeast infection at some point in their lives.
[b] Thin, yellow, foul-smelling discharge—Trichomonas, which is usually transmitted s*xually.
[c] Thin, gray or white, foul-smelling discharge—bacterial vaginosis.
[d] Pelvic inflammatory disease (PID). Frequently caused by STDs that infect the cervix, uterus, ovaries, or fallopian tubes, this is the most common and serious complication of an STD and occurs in 1 million women every year. Symptoms include va**nal discharge or bleeding, lower abdominal pain, and fever. Chronic PID can result from one or more infections. The most common identifiable causes are gonorrhea and chlamydia, both of which are s*xually transmitted. About 20 percent of women with PID become infertile.
[e] Ge***al herpes. This infection can produce va**nal discharge if it affects the cervix. The first episode of ge***al herpes also features fever, itching, headache, and general muscle aches.
[f] Infection of the inside of the uterus. This condition, known as endometritis, is usually caused by STDs, fibroid tumors, cancer, giving birth, or intrauterine devices (IUDs).
[g] Inflammation of the va**na due to lack of estrogen. As a woman enters menopause, her body produces increasingly erratic amounts of estrogen. This often causes the va**na to dry out and become irritated. The condition is known as atrophic vaginitis and is treatable by estrogen replacement therapy, va**nal creams, and va**nal suppositories.
[h] Other, less common causes of va**nal discharge include pregnancy, ge***al warts, cancer, and foreign objects in the va**na, such as a tampon that could not be removed.

Your doctor will ask you about the type of discharge and whether it occurs immediately before, after, or during menstruation or s*xual activity. You should also expect to undergo a pelvic exam.

The normal discharge doesn’t smell and does not cause any irritation or itching.
A discharge is likely to be abnormal if:
[1] it smells fishy
[2] it’s thick and white, like cottage cheese
[3] it’s greenish and smells foul
[4] there’s blood in it (except when you have a period)
[5] it’s itchy
[6] Do you have any ge***al sores or ulcers
[7] you have abdominal pain or pain during in*******se
[8] it started soon after you had unprotected s*x with someone you suspect could have a s*xually transmitted disease.

POSSIBLE CAUSES
[1]Bacterial vaginosis — Bacteria that normally live in the va**na overgrow, causing a grey discharge and fishy odor that worsens after s*xual in*******se. Bacterial Vaginosis is usually not s*xually transmitted.
[2]Cervical or va**nal cancer (rarely a cause of excess discharge)
[3]Chlamydia
[4]Forgotten tampon or foreign body
[5]Gonorrhea
[6]Other infections and s*xually transmitted diseases
[7]Trichomoniasis
[8]Vaginal yeast infection
PREVENTION
[1]Keep your ge***al area clean and dry.
[2]Avoid douching. While many women feel cleaner if they do**he after menstruation or in*******se, it may actually worsen va**nal discharge because it removes healthy bacteria lining
