Episiotomy is a surgical cut on the wall of the opening of the va**na given by the doctor or midwife during labour when the baby's head is visible and the woman pushing to expedite delivery.
It is given when it is obvious the woman will end up with an accidental tear. Local anaesthesia should be given to to prevent pain before the cut and repair of the cut after delivery. It is not true that the labour pain will mask the pain from the cut.

PELVIC ORGAN PROLAPSE
Pelvic organ prolapse refers to descent of one or more pelvic organs (Uterus, Urinary bladder, Re**um, Small bowel) beyond their normal position due to weakness of ligaments and muscles that supports them.
The weakness is usually due to injury sustained during va**nal delivery as the baby passes through the birth canal.
There is descent or prolapse of the pelvic organ initially with the va**na and eventually outside of the va**na.

Types:
The prolapse can be:
1. Utero-va**nal prolapse (Uterus)
2. Cystocele (Urinary bladder)
3. Rectocele (Re**um)
4. Enterocele (Small bowel)
5. Vault prolapse (In people who have previously had surgical removal of their uterus)

Causes:
1. High number of va**nal birth
2. Prolonged labour
3. Big babies
4. Neglected perineal tears
5. Menopause
Etc

Treatment
This depends on type of Pelvic organ prolapse, desire to have more babies, fitness for surgery etc.
1. Pelvic floor exercise can be done
2. Use of va**nal pessaries
3. Use of Oestrogen cream therapy
4. Surgery ( This is the mainstay of treatment)

Welcome to another week. We continue our study on the Kidneys.

Today we look at how they work. That is, their functions.

FUNCTIONS OF THE KIDNEYS

💥 Blood filtration:
Healthy Kidneys filter about 1/2 cup of blood per minute removing wastes and extra fluid.

💥 Urine formation:
The kidneys are important because they remove wastes and extra fluid from your body.

They also remove acids produced by the cells of your body as well as wastes from breakdown of medications you take.
All of these are then excreted and sent to the bladder as urine.

💥 Regulating and filtering essential Minerals from the blood:
The kidneys during filtration also extract useful and essential minerals needed by the body and return them back into circulation.

💥 Blood Pressure Regulation:
The kidneys produce a hormone that helps in the regulation of blood pressure.

💥 They also produce another hormone that helps in the production of new red blood cells. This hormone is known as Erythropoietin.

💥 The kidneys produce an active form of Vit D that help in the promotion of strong and healthy bones.

So these in summary are among the important functions carried out by the kidneys.

The Structure Of the Kidneys.

Welcome to another new week.

We continue our study on the kidneys by looking at the way the kidney is structured or built to carry out it's functions.

💥 The kidneys are bean shaped organs.

💥 The outer surface is covered by a tough fibrous tissue and fat that protects it from damage.

💥 Each kidney is approximately 3cm thick, 6cm wide and 12cm long.

Internal Structure.

Inside the kidney has two major layers.

💥The two major layers are Cortex which is the outer layer and the Medulla which is the inner layer.

There is a third part though known as the pelvis.

💥There are two major blood vessels that supply the kidneys. The renal artery and renal vein.

💥 The renal artery brings blood to the kidneys and the renal vein takes blood away from the kidneys. That is blood that has been filtered.

💥The NEPHRON is the main functional structure of the kidney as this is where the filtration takes place.

💥The Kidneys recieve about 25% of the total blood volume pumped by the heart which is about 1.2-1.3L

Welcome back from the Holidays.

So today we begin a series on the Kidneys.

We will be starting with a General Overview Of the Human Kidneys.

💥 The kidneys are a pair of bean-shaped organs.

💥 They are found on either side of the spine, below the ribs and to the back.

💥 Each kidney is about 4-5 inches long, that's about the size of the fist.

💥 The kidneys main job is to filter the blood.

💥 The instrument the kidney uses in filtration of the blood is called the NEPHRON.

💥 Each kidney has around 1 million Nephrons.

💥 The kidney is well supplied by major blood vessels known as the Renal arteries and veins. These carry blood into and away from the kidney for filtration.

💥 If blood stops flowing to the kidneys, a part or all of it would die.

So these are a few things to note about the kidneys.
