*Know If You’re Overweight; It Could Be The Only Health Problem To Be Treated To Treat Others*

The terms “overweight” and “obesity” refer to body weight that is greater than what is considered normal or healthy for a certain height.

The balance between calorie intake and energy expenditure determines a person's weight. If a person eats more calories than he or she burns (metabolizes), the person gains weight (the body will store the excess energy as fat). If a person eats fewer calories than he or she metabolizes, he or she will lose weight. Therefore, the most common causes of obesity are overeating and physical inactivity. Ultimately, body weight is the result of genetics, metabolism, medications, environment, behavior, and culture.

Body mass index (BMI) is a simple index of weight-for-height that is commonly used to classify overweight and obesity in adults. It is defined as a person's weight in kilograms divided by the square of his height in meters (kg/m2).

For adults, WHO defines overweight and obesity as follows:

An adult who has a BMI of 25-29.9 is overweight, and an adult who has a BMI over 30 is obese. A person with a BMI of 18.5-24.9 has a normal weight. A person is morbidly obese (extreme obesity) if his or her BMI is over 40.

Obesity is frequently subdivided into categories:

Class 1: BMI of 30 to < 35
Class 2: BMI of 35 to < 40
Class 3: BMI of 40 or higher.
Class 3 obesity is sometimes categorized as “severe” obesity.

Another easier method to tell if you’re at a healthy weight is from your waist size. A waist circumference greater than 35 inches in women and greater than 40 inches in men is said to be overweight.

“Waist circumference above these numbers indicates excessive belly fat, a dangerous type of fat surrounding vital organs, which increases one’s risk of diabetes, high cholesterol, high blood pressure, and the metabolic syndrome.”

To measure your waist circumference, all you need is a measuring tape.

“Place it on the top of your hip bone, bringing it around your body and level with your belly button”.

Resist the temptation to suck in and don’t make the tape too tight . An inaccurate reading only hurts you.

Worldwide obesity has nearly tripled since 1975.

In 2016, more than 1.9 billion adults, 18 years and older, were overweight. Of these over 650 million were obese.

39% of adults aged 18 years and over were overweight in 2016, and 13% were obese.

More than 1 in 3 adults were considered to be overweight.

About 1 in 13 adults were considered to have extreme obesity

39 million children under the age of 5 were overweight or obese in 2020.

Over 340 million children and adolescents aged 5-19 were overweight or obese in 2016.

In Africa, the number of overweight children under 5 has increased by nearly 24% percent since 2000.

Overweight and obesity are linked to more deaths worldwide than underweight. Obesity is preventable.

The major cause of overweight and obesity is poor diet and lack of physical activity - sedentary lifestyle.

Eating large amounts of processed or fast food – that's high in fat and sugar.

Drinking too much alcohol – alcohol contains a lot of calories, and people who drink heavily are often overweight.

Many sugary drinks – including soft drinks and fruit juice.

Large portions than you need – you may be encouraged to eat too much if your friends or relatives are also eating large portions drinking too.

Medications and Psychological factors - Diseases such as hypothyroidism, insulin resistance, polycystic o***y syndrome, and Cushing's syndrome are also contributors to obesity. Some medication side effects contribute to being overweight.

5 ways to know if you’re overweight

1.Waist measurement

“A waist circumference greater than 35 inches in women and greater than 40 inches in men.

2. Achy joints

Obesity is a risk factor for osteoarthritis, the most common type of arthritis. If you have achy knees or hips, or chronic back pain, you may be experiencing signs of a weight problem.

3. Snoring

“If your partner, best friend, or even you notice excessive snoring and you wake up restless, it may be a good reason to check your weight,”

4. Frequent heartburn

Changes in your body weight, even slight ones, can lead to more acid reflux.

5. If simple tasks, such as tying your shoes or cleaning a room, lead to fatigue, shortness of breath, or difficulty breathing, you may have a weight problem.

People with weight problems experience chronic inflammation because of the excess weight putting additional pressure on the organs, which leads to several chronic diseases such as;

Cardiovascular diseases (all heart disease including stroke and erectile dysfunction), which were the leading cause of death in 2012;

Diabetes mellitus;

Musculoskeletal disorders - Joint pains (especially osteoarthritis – a highly disabling degenerative disease of the joints);

Some cancers (including endometrial, breast, ovarian, prostate, liver, gallbladder, kidney, and colon).

Gallbladder disease and gallstones, breathing problems and gout and risks of infection.

The risk for these noncommunicable diseases increases, with increases in BMI.

Lets talk health.
Diabetes is on the ravage now.
It is more common now because of our diet and lifestyles.
Diabetes is dangerous, difficult to deal with and progressive.
It is not just enough to conrrol it, it keeps targetting the major organs and helps the patient to die from cardiovascular and other complications.
The solution to diabetes is PREVENTION.
Prevention measures:
1. Minimise sugary carbonated drinks.
2. Minimise carbohydrates or starchy meals.
3. Avoid eating too much. Cut down to half tank. You wont die. EAT MODERATE AND EAT HEALTHY.
More water to replace food.
4. Occasional and intermittent FASTING is good for our overral health
5. Regular PHYSICAL EXERCISES are beneficial. Avoid sedentry lifestyle!
6. Check your fasting blood sugar at least three times in a year.
**DO NOT OVERLOOK THIS FOR ANY REASON.

HOW A PREGNANT WOMAN FEEDS
Many times you hear a pregnant mother say, "doctor I am eating well, I ate three or four times yesterday".
This is a day-to-day disclosure by pregnant women in my prenatal clinic.
I am sorry I will disappoint you by saying you are under feeding when you eat twice or thrice or even only four times daily with a baby inside you.
Consider this, an average mature baby after birth feeds at least three to four times or more in AN HOUR. This translates to an average of 72 - 96 times in one day of 24hours knowing fully well that newborns and babies under one year do not know the difference between night and day. They eat at will.
Back to the feeding issue.
How come a pregnant woman feeds only three times daily till delivery and then she starts breast-feeding the same baby 72 times a day?
It shows us that many babies are underfed or starved while inside their mummies' wombs.
This is an awareness I often bring to the attention of my pregnant patients.
Anything less than 2 hourly feeding in the day which translates to at least six times per day is inadequate feeding for a pregnant woman. We need to bear in mind too that the capacity of the stomach of the mother reduces considerably throughout pregnancy. An average mother thus eats or fills at a sitting less than half of her stomach capacity before pregnancy: the more reason why the frequency of feeding must increase.
I often tell my patients two things:
1. A pregnant woman eats like a baby till her baby is born.
2. Do not say, I want to eat, say I want to feed my baby.
You don't need to put your own consideration above that of the baby.
You know about mood and appetite but the baby in your womb does not know these.
So consider the baby's welfare first. Eat for your baby.
Feed your unborn baby, she can't cry for food while inside.
Do not wait till you carry them in your hands or you hear them cry before realizing they need to be fed.
Feed them well while still inside.
It is for this reason born out of ignorance that maternal anemia in pregnancy which is a major contributor to perinatal morbidity and mortality is high.
The extreme cases are the pregnant women who engage in fasting and prayer in pregnancy.
I have often counseled my patients to reject in Jesus name any pastoral counsel that they should embark on fasting while pregnant.
Such extremes of ignorance certainly must have caused many intrauterine fetal deaths.
May God deliver us from the shackles of ignorance.
Remember to eat moderately and comfortably, balanced diets and fruits at least 6 times daily while you are pregnant.

REGISTRATION OF PATIENTS ON OUR ELECTRONIC PLATFORM

This is an issue that has brought unnecessary conflicts over the years up till the moment.
The conflicts are "unnecessary" because at the end of the day, with careful explanation and understanding, everyone became convinced that what we have done is fair and just.
Let me begin by stating that CORNERSTONE FOUNDATION HOSPITAL HAS DONE WELL AND IS AHEAD OF MANY OF ITS CONTEMPORARIES IN THE AREA OF RECORDS- KEEPING.
We started with paper files from the outset in 2006.
In the year 2017, we switched over to ELECTRONIC MEDICAL RECORDS (EMR) and till date we have remained there.
The advantages of EMR over paper files are many.
The world has changed and so CFH decided to FOLLOW the modern trend.
Just as you do not not need PASSBOOKS to run your bank account again, the same way you don't need HOSPITAL CARDS and NUMBER to access your records in the hospital.
All we need to know is YOUR NAME and your records are popped instantly on the screen. After all, you can't forget your name and if you forget your FAMILY must remember.
Therefore, time-wasting in attending to patients is minimized.
Therefore confusion and loss of medical records is eliminated.
So documentation, data retrieving, accounting, taxation, becomes easier.
One of the ideals we want to uphold is PROMPTNESS in responding to patients, and it is the key benefit of our EMR.
Investment on the EMR has paid off, and we do not need need extra physical space for rat and cockroach infested folders which take nerve - wrecking physical effort to locate sometimes.
However we keep having issues because patients are mostly used to the old manual records.
This is the reason why it often takes a lot of explanation for new patients to understand.
Some of the explanations are what I have set out to make here.
Most frequently encountered QUESTION: Family or individual cards/files?
ANSWERS:
We do not operate CARD system but FILES.
We run FAMILY FILES but EACH INDIVIDUAL in the families is registered in their family files to avoid MINGLING of information.
I must accept that the laziness and ignorance of our staff have largely been responsible for the conflicts we have had so far.
Laziness in doing meticulous proper job irrespective of the condition a patient is seen especially in emergency situations.
Our staff are in a hurry to see that a patient is seen especially when relations mount pressure on them at the registration point. They forget that it is better to spend time laying a solid foundation than to hurry to put a shaky one on ground.
They are poor communicators.
IN EMERGENCY SITUATIONS, AND AT ODD TIMES AND FOR SECURITY REASONS, PRIORITY SHALL BE ACCORDED MAINLY TO THOSE WHO ARE ON OUR REGISTER.
It is for this reason that we insist, ANYBODY CAN COME TO REGISTER HIS/HER FAMILY FREE OF CHARGE BEARING IN MIND THAT SICKNESS AND EMERGENCIES DO NOT GIVE APPOINTMENT.
It is unwise to claim, "I DON'T FALL SICK AND I DON'T GO TO HOSPITAL".
We appeal to everyone to just register and keep praying you won't need hospital services.
There are families we have registered for over five years without seeing their members. It still makes sense and costs them nothing.
You will only pay for your registration anytime you need our services (and that is only once) otherwise REGISTER YOUR FAMILY FREE.
Registering partially and excluding some members of your family is not helpful because nobody knows who and when the need medical attention comes up.
Once the name is not found in our records at odd times, we may not accept the patient because the process of registering patients may be time consuming and we do not want to waste time in emergency situations.
It is therefore wise to REGISTER AHEAD.
The rule now is "EVEN IF IT IS ANTENATAL REGISTRATION, THE WHOLE FAMILY MUST BE Registered".
WE DO NOT DO ISOLATED REGISTRATION AGAIN.
REMEMBER THAT REGISTRATION IS FREE UNTIL THE PERSON REGISTERED REQUIRES THE SERVICES OF THE HOSPITAL.
Tell your neighbors and new residents looking for medical facilities that this is what your hospital does.

Do you know?
That obstetric miscalculations can lead to fatalities and and potential image bashing litigations?
GPs who see a large turn out of obstetric cases are hereby enjoined to be very careful in this regard.
There needs to be active defense lines in place. Your midwives should be versatile and meticulous.
Remove the untrained from your obstetric lines of defense.
Be extra meticulous especially nowadays that many patients don't remember their LMP and rely absolutely on ultrasound dates. Remember that your palpation may sometimes fail you.
It is always a good practice to take some time to review the calculations once the patient is or perceived to be 36 weeks GA.
At that stage you can detect miscalculations and address them.
Often times when this is not detected by 38 and 40 weeks the chances of fatality becomes very high.
Benefits.
The patient is saved from:
1. Premature and preterm deliveries and their attendant complications
2. Post term IUFDs from placental insufficiency.
3. Unnecessary Caesarian sections.
This is pertinent in situations where neonatal care is rarely available in most centers.
