Part 9: Urethral Stricture.

The small pipe that passes through the peenis and conducts urine from the urinary bladder to the outside is known as the urethra. The narrowing of this small pipe is known as urethral stricture. It is much more common in men.

It can result from injury to the urethra with abnormal healing processes and scarring. The injuries can be from falls onto the scrotum/perineum or from the insertion of materials such as medications, catheters, or scopes into the urethra.

It can also result from the damages caused by poorly treated s*xually transmitted infections such as gonorrhoea, chlamydia, and others. Such infections may have occurred several years earlier, probably when the patient was in his 20's

The affected individuals may have difficulty passing urine, poor urinary stream, or have the urine stream divide and spray in different directions , like a watering can People with this problem may have similar complaints as those with an enlaged prostate.

So, it is very important they see their doctor early who may refer them to the urologist for proper evaluation and treatment.

Is The Game Really Worth The Candle?

A few years ago, I attended to a young man in his early 20's, who had copious discharge of pus from his peenis.

I asked him to tell me how he got to that point and he told me his story:

In his community and culture, premarital xex was regarded as an abominable act, and such discussions are not held in the open.

So, as with many of the young boys of his age, he drove more than 6 hours to a neighbouring community to have firsthand experience. .

He spent three days there, and each day he had in*******se with some "commercialized" women. Each day, he paid some significant sum for just an hour affair. He even had to pay much more than routine, for desiring not to use any form of protection. You can imagine! According to him, each episode of the real act did not last more than 10 minutes, the rest was for onboarding and offboading.

He repeated the act for each of the the days, and made sure he satisfied his fantasies of shape, size, and colour. In addition to this, he made sure he tried out some substances too, after which he drove another 6 hours back to his base. For him that was a vacation well-spent.

A few days later, he developed this painful urination, difficulty passing urine, and discharge of pus from his chief of staff.

With the culture of non-discussion, he had a few more days of delay before seeking help in the hospital.

As I looked at him, I wondered how many men that may have passed through sumilar difficult conditions and expenses, just to have an interaction with this so called mucous membrane.

Many times, I have read about tales of men who had to give up fortunes, kingdoms, integrity, family just to have a similar "fleeting feel". I have also heard men talk about how they have had to kneel for hours begging, run errands, jump fences at night, keep vigil, and other humiliating things, just to have a short-lived dip in this pot, which some authors have describe as a "singular vessel for manipulation".

Back to this young man, with this STI, if he is not properly treated, is at risk of developing serious complications such as infertility and urethral strictures later in life.

Now, the question: is this xex game really worth the candle?

Part 9 : Erectile Dysfunction. (ED)

Poor er****on can cause significant distress and strain in self esteem, marriages and relationships.

Contrary to what is being propagated by some herbalists in Nigeria, poor er****on is not caused by "staph" or "staphylococcus " or any s*xually transmitted infection.

It occurs when when a man is unable to achieve, or maintain firm er****on sufficient for in*******se.

ED can results from psychological problems related to physical or emotional stress, depression, fear, anxiety, relationship problems, low self esteem, and other socioecinomic stressors.

It may also be a manifestation of some underlying medical conditions such as vascular disease, low testosterone, diabetes, thyroid disease, hypertension, heart disease, Obesity, abnormal cholesterol, sleep disorders, and some neurological diseases.

In addition, some men who may have had prior pe**le fracture, or previous pelvic surgeries may have difficulty achieving er****on again. Also, men who have problems of premature Ej*******on may have a problem restarting their er****on after a premature ej*******on..

It may also result from to***co use, drugs, ma*****na, alcoholism, some medications, and the abuse of some er****on- stimulating supplements and herbal preparations.

People who take herbal preparations that induce sustained and prolonged er****on of more than 4 hours may cause irreversible damage to their organ and may be at risk of having priapism- induced erectile dysfunction.

It is is very important that individuals with ED to start by maintaining healthy weight, having good sleep, exercise, stress management, healthy lifestyle, and seeing their doctor for detailed evaluation and treatment.

Part 8 : Premature Ej*******on.

Premature Ej*******on (PE) is one of the conditions that can cause significant distress for a man and/or his partner.

It is said to occur when the man ej******es in less than 2 minutes of going in, and this occurs (almost) ALL THE TIM and in all circumstances.

It is a clinical condition, though the exact cause is not clear. However, it is known to have both psychological and biological factors contributing to the problem.

It is not caused by s*xually transmitted infections, contrary to the lies propagated by some herbalists in Nigeria.

For most patients, it can be completely treated with medications, counselling and some behavioural techniques (especially for married couples).

Therefore, for couples to whom PE is a recurrent problem, and/or interferes with achieving pregnancy, they should endeavour to see their primary care doctor, who can coordinate the care with urologists, and psychotherapists.

The problem can be fixed, so see competent care.

Part 7 : Medications and Low S***m Count.

Men who are on evaluation or treatment for low s***m count should pay attention to this.

Some of these medications can cause low seminal fluid volume and/ or low s***m count in men.

Prostate medications such as Tamsolusin, finasteride, and dutasteride,

Antidepressants such as paroxetine, fluoxetine, and sertraline,

Cancer medications such as cisplastin, vincristine, and doxorubicin,

Opiods such as morphine, codeine, and he**in,

Exogenous testosterone and muscle building (anabolic) steroids

Antifungal meds such as ketoconazole.

Antiepilepsy medications such as carbamazepine, and valproate

Other medications include, spironolactone, cimetidine, colchicine, and sulfasalazine.

Heavy alcohol use, ma*****na to***co use, and methamphetamine (mkpurummiri) can also decrease s***m count.

Some herbal preparations and some "s*x drive" supplements can also cause low s***m count.

So, if you are on evaluation or treatment for low s***m count, you may need to discuss with your doctor on alternative medications.

Part 6: Low s***m count

Low s***m count is used to describe a situation where the semen produced by the man contains fewer than the expected number of s***m cells., (also known as oligos***mia).

It is diagnosed when the count is less than 15 million s***m cells per milliliter of seminal fluid.

This is determined through the test called seminal fluid analysis.

It may result from chromosomal abnormalities, hormonal imbalance, emotional stress, depression, varicocele, and groin surgeries.

It may also result from Obesity, to***co, heavy alcohol use, some medications, and exposure to radiation or some chemicals.

Also, overheating your testicles through hot tubs, or sitting on them for long hours while working or driving can also decrease s***m count.

Infections such as gonorrhoea and/ or Chlamydia, can also affect s***m count.

Some of the causes can be treated.

The truth is, some men have Low s***m count, but are still able to father a child, probably because, they may have a good proportion of actively moving, good-quality s***m cells, or it may be because they were lucky to marry a woman with high fecundity.

So, unless a man has zero counts, there is still a chance that he can achieve pregnancy.

Therefore, couples who have problems achieving pregnancy after one year of living together, and having regular coitus should endeavour to see their doctor. There are always treatment options.

Part 5 : Watery Spe*m.

This is another reason some men are being abused and humiliated in marriages and relationships.

I have attended to couples who had infertility problems.

The woman would almost always state that ^after meeting with the husband, the whole thing pours out whenever I comes out of the bed".and she would accuse the man of having watery spe*m. She may even conclude that the man has infections that needs to be treated. This has caused alot of problems for couples.

The fact is that nomatter how thick tbe semen is, it is expected to liquefy within 30 minutes, failure to do so, is also not good .

It is also a fact that it is expected that greater proportion of what a man deposits in the woman is likely to flow out when she comes out or stands.

So, if it flows out as a lighter liquid afterwards, that does not mean that the man has "watery spe*m". So it is unnecessary to accuse the man of having watery spe*m.

What is more important is whether the cells are well-formed, in adequate amount, and are actively moving.

To be sure, one has to do the test called seminal analysis.

It is very important to disccus the result with your doctor.

Part 4 : Small size Peenis.

Some years back, I attended to a 19-year old boy who was battling with low self-esteem because of his pe**le size.

The guy was almost having body dysmorphic disorder/ small peenis syndrome.

He said that his fiends were making mockery of him because of the size of his peenis. They even told him that his girlfriend would leave him if she discovered that he was not endowed.

So, I examined him thoroughly, and reassured him that he had normal pe**le size.

I also attended to many others Later.

Each time I wonder how may male children who have been made to believe, that they are genitally inadequate.

Micropen*s is tbe term used to describe sizes that are less than the size expected of a given age.

It is a rare condition, which may be found in 1 out of 10, 000 births.

It usually results from the abnormalities in the production and function of the male hormon, testosterone or due to genetics ( as per in your parents)

Parents who are concerned at birth about the size of the child's peenis should get their doctor examine before they leave the hospital. We measure the Stretched Pe**le Length (SPL)

For a new born, a stretched pe**le length (SPL) of 0.75 inches (approx. 2cm) or less, indicates small peenis, and treatment may be needed as soon as possible.

Young boys who also feel that their size is not adequate should also see their doctor on time, so that thenboy will not grow up battlingbselfbimage. There are different ranges for different ages till adulthood.

If the diagnosis is made very early, some correctional treatment can be given to improve the size. Sometimes, surgery may be required, and
Pediatric (urologic) surgeon is usually the person to do it.

However, once, the person has attained adulthood, treatment may not achieve much.

Note that there is no, evidence to support that exercise, medication, pills, herbs, or cream increase the pe**le size once adulthood has been attained. So don't go about wasting your time and money on creans and herbal remedies.

Again, most people that their peers and partners label as having small size peenises actually have normal range sizes.

If not sure talk to us

For an adult above 21 years, any peenis that is more than 3.6 inches or 9.3cm on full stretch (SPL) should be seen as normal. Full er****on also correlates with SPL.

Provided it is normally structured, the size, does not necessarily affect the individuals urinary and s*xual functions. Skills make the difference.

So, do not let any one trouble you because of your pe**le size, especially if it is within normal range.

Part 3: undescended te**is.

Do men actually have two te**es or one?

In the second year of our medical school, a teacher was discussing the development of the te**es and how they travel from the abdomen to the sukrutum, where they eventually reside. One of our very innocent late-teens classmates asked with some curiosity; do men actually have two te**es or one?

It was an interesting moment.

The fact is, men actually have two te**es, but sometimes, one or both of the te**es may be arrested on their way to the sac, and not present at birth. That is called undescended te**es.

The undescended te**is may be found in the groin or even way up in the abdomen.

The exact cause is unknown.

Premature babies or abies born to women who have diabetes, or smoke, or use alcohol have higher chances of having undescended te**es. There may be also such history in the family.

It is important that those who assess new born at birth confirm that the two te**es are in their apartments. Parents can check too, at home

If one or both are missing at birth, or at any time, there is no cause for alarm. Be sure you inform your doctor, anyway.

The migration of the undescended te**es should be monitored monthly until the child is about 6 months. Often times they descend to the sac on their own or with medical treatment.

However, if the te**is does not descend up to 12 months of age, then surgery is usually the best option.

It is best to do the surgeryon or before before the first birthday, or at most 18 months of age.

An undescended te**es that is not corrected, is increases the chances poor fertility, and the development of cancer of the te**es later in life
