Pyloric stenosis is a condition in infants where the pylorus (the muscle between the stomach and small intestine) becomes abnormally thickened, blocking food from moving into the intestines.

🔍 Key Facts:
• Age of Onset: Typically seen in babies 2–8 weeks old
• More common in: Males, firstborns, and formula-fed infants

🚨 Signs & Symptoms (Classic Triad):
1. Projectile vomiting – forceful, non-bilious vomiting after feeding
2. Olive-shaped mass – felt in the right upper abdomen (hypertrophied pylorus)
3. Visible peristalsis – waves moving across the baby’s belly before vomiting

🧪 Diagnosis:
• Ultrasound – gold standard; shows thickened pyloric muscle
• Lab findings:
• Hypochloremic, hypokalemic metabolic alkalosis due to vomiting

💉 Treatment:
• IV fluids and electrolyte correction first
• Surgical repair: Pyloromyotomy – cutting the muscle to relieve obstruction

Prolonged labour and obstructed labour are significant obstetric complications that can lead to serious maternal and fetal outcomes if not managed promptly.

💟1. Definitions:

Prolonged Labour:

🎈Prolonged labour is labour that lasts longer than expected due to slow cervical dilation or delayed descent of the fetus, usually defined as:
• >20 hours in primigravida
• >14 hours in multigravida

Obstructed Labour:

🎈Obstructed labour occurs when there is no progress in labour despite good uterine contractions, due to a physical blockage that prevents the fetus from descending through the birth canal.

💟2. Causes:

🎈A. Prolonged Labour Causes:
• Power – Inadequate uterine contractions
• Passenger – Large baby, malposition (e.g., occiput posterior), fetal anomalies
• Passage – Contracted pelvis, soft tissue obstruction

🎈B. Obstructed Labour Causes:
• Cephalopelvic disproportion (CPD)
• Malpresentations (e.g., brow, face, transverse lie)
• Pelvic tumors
• Cervical stenosis or scarring
• Shoulder dystocia

💟3. Signs and Symptoms:

🎈For Prolonged Labour:
• Labour not progressing after several hours
• Slow or no cervical dilation
• Maternal exhaustion and dehydration
• Fetal distress

🎈For Obstructed Labour:
• Strong, frequent contractions without descent
• Bandl’s ring (abnormal uterine contour)
• Distended bladder
• Foul-smelling vaginal discharge (in prolonged cases)
• Signs of fetal distress or intrauterine death
• Maternal distress, dehydration, infection

💟4. Diagnosis:
• Vaginal examination: Lack of cervical dilation or descent
• Abdominal exam: Palpable Bandl’s ring, uterine tenderness
• Fetal heart monitoring: Signs of distress or bradycardia
• Partograph: To track labour progress

💟5. Management:

🎈Prolonged Labour Managements:
• Augmentation with oxytocin (if contractions are weak and no obstruction)
• Amniotomy (if membranes are intact)
• Supportive care (hydration, pain relief, emotional support)

🎈Obstructed Labour Managements:
• Immediate cesarean section (preferred option)
• If fetus is dead and vaginal delivery is necessary:
• Destructive procedures (e.g., craniotomy) in specific settings
• Resuscitate mother (IV fluids, antibiotics if infection suspected)
• Monitor for complications: PPH, sepsis, uterine rupture

💟6. Complications:

🎈Maternal Complications:
• Uterine rupture
• Postpartum hemorrhage (PPH)
• Fistula formation (Vesicovaginal or rectovaginal)
• Sepsis
• Maternal death

🎈Fetal Complications:
• Hypoxia/asphyxia
