PREVENTING HEART DISEASE.

We have three ways of preventing heart disease, secondary way , primary and primordial prevention.

Secondary prevention.
These effect are started after someone has a heart attack or stroke, undergoes angioplasty or bypass surgery, or develops some other form of heart disease. It involves taking medication like aspirin or a cholesterol lowering statin, quitting smoking and losing weight if overweight and following a healthy diet.

Primary prevention.
Primary prevention aims to keep an individual at risk of heart disease from having a first heart attack or stroke, needing angioplasty or surgery or developing some other form of heart disease. Primary prevention is usually aimed at people who already have developed cardiovascular risk factor such as high blood pressure or high cholesterol.

Primordial prevention.
The word primordial means existing from the beginning primordial prevention involves working to prevent inflammation, atherosclerosis and endothelial dysfunction from taking hold and thus prevent risk factor such as high blood pressure, high cholesterol , excessive weight and. Ultimately cardiovascular events.

Steps for the primordial prevention of heart disease.
Not smoking.
Maintain a healthy weight.
Exercise.
Follow a healthy diet.

SOME LIFESTYLE THAT CAN BE DANGEROUS TO THE KIDNEY.

The kidney play a central role in exception of many metabolic breakdown products, including ammonia, urea and creatinine from protein, and uric acid from nucleic, drugs and toxins. They also regulate fluid and electrolyte balance. This is achieved by making large volume of an ultrafiltrate of plasma (120ml/min, 170L/day) at the glomerulus and selectively reabsorbing component of this ultrafiltrate at points along the nephron.

The kidney also regulate acid-base homestatasis , calcium and phosphate homeostasis, vitamin D metabolism and production of red blood cells. They are also important in regulating blood pressure.

Some lifestyle that can be dangerous to our kidney.

High salt intake.
It is recommended that every human should not exceed salt intake more than 2g per day , how ever some individual do add additional salt to their diet which can led or cause some damage to the kidney reduction in sodium can be a way of reducing the risk of kidney disease.

Regular intake Analgesics.
Excessive intake of pain killers drugs can have a negative effect on the kidney, taking this drugs on a long term can lead to chronic interstitial nephritis which is an inflammation of the nephron of the kidney.

Excessive intake of protein diet.
Eating too much of processed meat, cheese and other high protein based food can lead to Kidney problem.

Excessive alcohol intake.
Too much alcohol might be dangerous to the kidney, this can increase uric acid in the urinary tubules and cause some certain abstraction.

Not emptying the bladder.
These usually occur in older patient and dementia patient, holding on urine for a long time without emptying it can lead t UTI which can lead to kidney damage over time.

Ignoring common infection.
Common infection such as urinary tract infection can lead to kidney disease, please consult your doctor in case of UTI.

Lack of sleep.
Sleeplessness can result in increase of blood pressure and can cause arteriosclerosis, high blood pressure can also be a leading cause of kidney damage.

LIVER DISEASE AND DIETARY MANAGEMENT.The liver is undoubtedly one of the most important organs of the human body, located in the upper right quadrant of the abdomen. The liver is responsible for multiple functions including metabolic and synthetic functions. Therefore, it is essential to note all the warning signs of liver damage.
Things to avoid and reverse liver damage:
- Avoid alcoholic beverages
- Maintain a healthy weight and exercise regularly
- Avoid smoking
- Avoid unprotected s*x
- Do not over consume fat soluble vitamin supplements

DIABETES IN PREGNANCY.During pregnancy, maternal glucose metabolism changes to optimise glucose and other nutrient delivery to the fetus. This is apparent in the second half of pregnancy, when there is an increase in maternal tissue insulin resistance, such that glucose is preferentially supplied to the fetus rather than maternal tissue. This is largely driven by the maternal hormonal environment, with increased progesterone and oestrogen and in particular, human placental lactogen. The delivery of the placenta results in rapid decline in human placental lactogen with a rapid reversal of insulin resistance soon after birth. During pregnancy, fasting plasma glucose decreases slightly, while post-prandial blood glucose may be increased. In the fetus , insulin secretion is driven by fetal blood glucose levels, which are determined by the maternal glucose concentration. Thus maternal hyperglycemia drives fetal hyperinsulinaemia . Since insulin is a major fetal growth factor, hyperinsulinaemia in turn drives increased fetal growth, resulting in increased birth weight.

DIABETES AND CHRONIC KIDNEY DISEASE.
Dietary approach in special group of chronic kidney disease patient.
Diet and lifestyle modification are key component of diabetic care and optimal nutritional approach helps the management of blood glucose, lipid, body weight and blood pressure. Thus reducing both the incidence and progression of diabetic complication, including chronic kidney disease.

The Italian Guidelines for diabetic care suggest that people at high risk of diabetes should be encouraged to follow a diet rich in fiber from vegetables, fruits and whole grain and low animal fat .

The first step in managing a diet for a diabetic patient in an early stage of chronic kidney disease is to ensure that carbohydrates intake is within the recommended range of 45%-60% of total calories and the actual fiber intake is similar to that of the general population 14g/1000kcal per day.
Dietary sodium and protein restriction should be a mainstay in the nutrition therapy of all patient with proteinuria and it has been proven effective even in overt diabetic nephropathy when chronic kidney disease progresses a high protein intake is detrimental for the progressive decline of renal function .

Diabetes is the cause of kidney failure and the leading cause of micro albumin in the urine over time, that extra sugar damages the small blood vessels in your kidneys. It becomes harder for them to clean your blood. Diabetes can also harm your nerves which may lead to kidney injury.
