EAR: OTITIS MEDIA

Otitis Media is an inflammation or infection of the middle ear β€” the air-filled space behind the eardrum that contains tiny vibrating bones of the ear. It is especially common in children but can also affect adults.

TYPES OF OTITIS MEDIA

1. Acute Otitis Media (AOM):

Sudden onset

Caused by bacteria or viruses

Symptoms include ear pain, fever, irritability (in children), and hearing loss

2. Otitis Media with Effusion (OME):

Fluid remains in the middle ear after the infection clears

Usually painless but may cause muffled hearing

3. Chronic Otitis Media:

Persistent or recurring infection

May involve persistent discharge from the ear (otorrhea)

Risk of eardrum perforation

CAUSES AND RISK FACTORS

Bacterial or viral respiratory infections (like colds)

Eustachian tube dysfunction

Allergies

Exposure to cigarette smoke

Bottle-feeding in infants (especially while lying down)

Age (more common in children under 6)

Family history

SYMPTOMS

Ear pain or discomfort

Hearing loss or muffled hearing

Fever

Fluid drainage from the ear

Irritability, crying, or trouble sleeping (in children)

Loss of balance

DIAGNOSIS

Otoscope exam: to visualize the eardrum

Tympanometry: to assess eardrum movement

Hearing tests: in recurrent or chronic cases

PREVENTION TIPS

Breastfeeding

Avoiding bottle-feeding while lying down

Reduce pacifier use

Avoid secondhand smoke

Vaccinations

CERVICAL SPONDYLOSIS

Cervical spondylosis is a common, age-related condition that affects the joints and discs in your cervical spine (neck). It is also known as neck arthritis or cervical osteoarthritis.

πŸ” CAUSES

Aging: Most common cause due to wear and tear of cartilage and bones

Dehydrated spinal discs: Discs lose water and become less cushiony

Herniated discs: Bulging or slipped discs can press on nerves

Bone spurs: Extra bone growths can compress nerves

Ligament stiffness: Neck ligaments may become stiff over time

⚠️ SYMPTOMS

Neck pain and stiffness

Pain that radiates to shoulders, arms, or hands

Numbness or tingling in arms, hands, or fingers

Weakness in muscles

Headaches, especially at the back of the head

Loss of balance or difficulty walking (if spinal cord is compressed)

🩺 DIAGNOSIS

Physical exam

X-rays

MRI or CT scan to assess nerves and discs

EMG (Electromyography) if nerve damage is suspected

🧘 Self-care Tips

Maintain good posture

Sleep with supportive pillow

Avoid carrying heavy bags on one shoulder

Gentle neck exercises (as advised by a physiotherapist)

GE***AL WARTS

Gl
Ge***al warts are soft growths that appear on the ge****ls, a**s, or surrounding skin. They are caused by certain strains of the human papillomavirus (HPV), particularly types 6 and 11, which are considered low-risk (not cancer-causing).

CAUSES

Human Papillomavirus (HPV) – a s*xually transmitted infection

Spread through:

Vaginal, a**l, or oral s*x

Skin-to-skin contact with an infected person

Even without visible warts or symptoms

SYMPTOMS

Small, flesh-colored or gray swellings

Clusters that resemble cauliflower

Itching, burning, or discomfort in the ge***al area

Bleeding during in*******se (sometimes)

> Locations in females: V***a, cervix, va**na, a**s
Locations in males: P***s, sc***um, a**s, urethra

DIAGNOSIS

Physical examination

Colposcopy (for internal ge***al warts in women)

Biopsy (if lesions appear unusual or don’t respond to treatment)

PREVENTION

HPV vaccination (e.g., Gardasil 9) – protects against high- and low-risk HPV strains

Condom use – reduces risk, though not completely protective

Limiting number of s*xual partners

Regular STI screenings

Well-defined, round or oval lump

Firm, rubbery texture

Painless

Moves easily under the skin when touched

Usually single, but some women may have multiple fibroadenomas

CAUSES & RISK FACTORS

Exact cause is unknown.

Likely linked to hormonal changes, particularly estrogen.

May become larger during pregnancy or smaller after menopause.

DIAGNOSIS

1. Clinical breast exam

2. Ultrasound – especially useful for women under 30

ELECTROCARDIOGRAM (ECG)

An Electrocardiogram (ECG or EKG) is a quick, painless, and non-invasive test that measures the electrical activity of the heart. It helps doctors assess the heart’s rhythm, detect heart conditions, and monitor treatment effectiveness.

WHAT AN ECG SHOWS

1. Heart rate – Too fast, too slow, or irregular.

2. Heart rhythm – Normal sinus rhythm or arrhythmias (e.g., atrial fibrillation).

3. Electrical conduction problems – Delays or blocks in heart's electrical system.

4. Heart attack (past or current) – Changes in ECG waves can suggest damage.

5. Heart chamber enlargement – Due to high blood pressure or heart disease.

6. Electrolyte imbalances – e.g., potassium or calcium levels affecting the heart.

HOW IT'S DONE

Electrodes (small, sticky sensors) are attached to the chest, arms, and legs.

Wires connect the electrodes to the ECG machine.

The test lasts about 5–10 minutes.

A 12-lead ECG is the most common type, recording 12 views of the heart.

WHY IT'S ORDERED

Chest pain

Palpitations or dizziness

Shortness of breath

Monitoring effects of heart medications

Routine check in patients with risk factors (e.g., hypertension, diabetes)

STRESS

Stress is your body’s natural response to any demand or challenge. It can be positive (e.g., helping you meet a deadline or perform well) or negative (when it becomes overwhelming or constant).

TYPES OF STRESS

1. Acute Stress

Short-term, immediate reaction to a threat or challenge.

Example: A near car accident or job interview.

2. Chronic Stress

Long-term stress from ongoing issues like work pressure, financial problems, or relationship difficulties.

Can be harmful to physical and mental health if not managed.

CAUSES (STRESSORS)

Work or school pressure

Relationship conflicts

Financial struggles

Health problems

Major life changes (e.g., moving, losing a loved one)

Uncertainty or lack of control

HOW IT AFFECTS THE BODY

When stressed, your body activates the "fight or flight" response:

Hormones like adrenaline and cortisol are released.

Heart rate and blood pressure increase.

Muscles tense up.

Breathing becomes rapid.

This is helpful in emergencies, but constant stress wears down your body and mind.

WARNING SIGNS OF STRESS

Frequent headaches

Trouble sleeping

Anger or irritability

Feeling overwhelmed

Loss of motivation

Changes in appetite

TIPS TO MANAGE STRESS

Exercise regularly (even a walk helps)

Sleep well

Eat healthy

Talk to someone you trust

Practice relaxation (deep breathing, prayer, meditation)

Take breaks and avoid burnout

Stay organized (to reduce last-minute pressure)

DEEP VEIN THROMBOSIS (DVT)

Deep Vein Thrombosis (DVT) is a condition where a blood clot (thrombus) forms in a deep vein, usually in the legs or pelvis. It can be life-threatening if the clot dislodges and travels to the lungs, causing a pulmonary embolism (PE).

CAUSES / RISK FACTORS

Prolonged immobility (e.g., long flights, bed rest)

Surgery, especially orthopedic (hip/knee)

Trauma to a vein

Pregnancy

Obesity

Smoking

Cancer or chemotherapy

Hormone therapy (e.g., oral contraceptives, HRT)

Inherited clotting disorders (e.g., Factor V Leiden)

SIGNS AND SYMPTOMS

Swelling in one leg (rarely both)

Pain or tenderness (especially in the calf)

Warmth over the affected area

Red or discolored skin

Leg cramps, often starting in the calf

> ⚠️ Some people may have no symptoms.

DIAGNOSIS

D-dimer test (blood test for clot breakdown products)

Doppler ultrasound (main imaging test)

Venography (rarely used, invasive)

CT or MRI venography (in complex cases)

COMPLICATIONS

Pulmonary embolism (PE) – life-threatening

Post-thrombotic syndrome (PTS) – chronic leg pain, swelling

Recurrent DVT

PREVENTION

Early mobilization after surgery or illness

Leg exercises during long travel

Use of compression stockings

Prophylactic anticoagulants in high-risk patients

Lifestyle changes: stop smoking, lose weight

SHOULDER DYSTOCIA

Shoulder dystocia is an obstetric emergency that occurs during va**nal delivery when the baby's anterior shoulder gets stuck behind the mother's p***c bone after the head has been delivered. This prevents further delivery unless specific maneuvers are performed. It can lead to serious complications for both mother and baby if not managed quickly and correctly.

CAUSES AND RISK FACTORS:

Although it can occur unpredictably, certain factors increase the risk:

Fetal macrosomia (baby > 4.0–4.5 kg)

Maternal diabetes

Obesity

Post-term pregnancy

Prolonged second stage of labor

Instrumental delivery (e.g., forceps or vacuum)

Previous shoulder dystocia

SIGNS:

"Turtle sign" – the fetal head delivers and then retracts against the perineum

Difficulty delivering the shoulders despite standard traction

Failure of the shoulders to descend

NEONATAL COMPLICATIONS:

Brachial plexus injury (e.g., Erb’s palsy)

Clavicle or humerus fracture

Hypoxia or brain injury

Stillbirth (rare in well-managed settings)

ACHONDROPLASIA

Achondroplasia is a genetic disorder that results in dwarfism, specifically short-limb dwarfism. It is the most common form of disproportionate short stature.

πŸ”¬ CAUSES

Caused by a mutation in the FGFR3 gene (Fibroblast Growth Factor Receptor 3).

The gene mutation affects the conversion of cartilage to bone, especially in the long bones.

Most cases are due to new (spontaneous) mutations, but it can also be inherited in an autosomal dominant manner.

πŸ‘Ά CHARACTERISTICS AND SYMPTOMS

Short stature: Average adult height is around 4 feet (122 cm).

Short arms and legs, particularly the upper arms and thighs (rhizomelic shortening).

Large head with a prominent forehead (frontal bossing).

Flattened nasal bridge.

Normal intelligence.

Bowed legs.

Limited range of motion at the elbows.

Spinal issues such as:

Lordosis (sway back)

Kyphosis (humpback)

Spinal stenosis (narrowing of spinal ca**l)

Delayed motor milestones in infancy due to hypotonia (low muscle tone).

🧬 INHERITANCE PATTERN

Autosomal dominant: One mutated copy of the gene is enough to cause the disorder.

If one parent has achondroplasia: 50% chance of passing it to the child.

If both parents have achondroplasia: 25% chance the child will inherit a severe form (often fatal), 50% chance will inherit the condition, 25% chance will be unaffected.

🩻 DIAGNOSIS

Prenatal: Ultrasound can detect short limbs; DNA testing can confirm.

Postnatal: Clinical features, X-rays, and genetic testing.
