What is first trimester screening ?

Its a prenatal test done between 11 to 14 wks ( 13 wks 6 days) to quantify the mother's risk of having a baby with some chromosomal abnormality particularly Down syndrome ( Trisomy 21) , Edwards syndrome ( Trisomy 18) and Patau syndrome ( Trisomy 13).

This test also predicts about chances of having pre eclampsia, IUGR, preterm birth later in pregnancy.

The placement of a transvaginal probe in the umbilicus improves resolution in obese pregnant patients with better transmission of ultrasound through the thickened abdominal wall and more satisfactory imaging of the fetal heart.

The technique, involves filling the umbilicus with ultrasound transmission gel and inserting the transvaginal probe into the umbilicus.

Nuchal translucency (NT) is the assessment of the amount of fluid behind the neck of the fetus, also known as the nuchal fold. An anechoic space should be measured sonographically in all fetuses between the 11 week to 13 week 6 days of the pregnancy (Figure ). Underlying pathophysiological mechanisms for nuchal fluid collection under the skin include chromosomal anomalies, cardiac dysfunction, venous congestion in the head and neck, altered composition of the extracellular matrix, failure of lymphatic drainage, fetal anemia or hypoproteinemia and congenital infection . This abnormal accumulation of nuchal fluid usually decreases after the 14th week.
