The importance of wearing gloves while cycling/riding motorbike in winter! The patient was cycling without gloves in this cold weather, his fingers became numb and couldn't apply the brakes in time. Lucky that he got away with just PIP joint dislocation of the middle and ring fingers, which were reduced in the Er itself.

Finally, my 1 year Felllowship in Hand and Upper Extremity Surgery at Max Smart Superspeciality Hospital, Delhi is complete and with that, a new journey as a Hand & Upper Extremity Surgeon has begun.

Proud to be the first Hand Fellow to be awarded a Gold Medal by the Hand to Shoulder unit.

I am very much thankful and indebted to my mentor, Dr. Vikas Gupta for everything that he has taught me during my Fellowship program. His teachings have been invaluable. He has been an ideal mentor that every apprentice yearns for.
His guidance and expertise have profoundly impacted my journey. Him sharing his knowledge, experiences, encouraging my growth, and inspiring me to strive for excellence has made me feel incredibly fortunate to have him as my mentor. His dedication to the art of healing and to the patients is truly admirable.

I feel really blessed to have had the opportunity to see so many variety of cases coming from not just all over India, but from all over the world to seek his expert opinion. Surgical procedures these days can be learnt from books, journals and even youtube. But to know which procedure to do in what kind of problem is the most trickiest and challenging part. I feel lucky enough to have had the opportunity to witness and participate in discussions regarding these complex cases with Dr. Vikas Gupta and Dr. Akram Jawed and assist them during those surgeries.

Dr. Akram Jawed has been another influential person during my stay. His vast and deep knowledge always inspired me to gain as much knowledge. It kept me putting in that extra effort to go through the books even after a tiring day.

I would also like to thank all my co-fellows Dr. Pallav, Dr. Hiba Dr. Sagar for helping me throughout my time and making this period not just about long working hours, research, studying, surgeries, procedures but also about having a good time as well.

All the staffs, Deepak ji, Zuby ji, Nikky ji, Sutish ji, Hitesh ji, Fahad brother, Anuj brother, Lophro sister, Sylvia sister, Divya Sister, Aditee sister, technicians - Kounan bhaiya, Vikky bhaiya, Amit bhaiya, Manish - all who made my stay really pleasant, were always supportive, warm and did not make me feel like I was away from home, thank you all for your hospitality and kindness.

I do hope I get to meet these wonderful people often in the future and I welcome them to Nepal whenever they get the opportunity.

Osteotomy and plating done on a patient with malunion of 5th metacarpal fracture that was initially treated with K-wires.

Outcome of plating (mini-plates) done on a young ptn who had fractures of the 3rd and 5th metacarpals of his right hand.
In patients with multiple metacarpal/phalangeal fractures, plating has a huge advantage over conventional treatments like putting a plaster or k-wires, apart from the scar that heals by 2 weeks.
When treated with plaster or k-wires, there is always the risk of displacement in future before the fracture heals. To try and prevent this, the fingers are immobilized for a long time which will ultimately lead stiffness of the joints. Whereas after plating them, you can start early mobilization, there is very little chance of stiffness and the patient will have a good grip.
