Today morning we had a patient with acute anterior wall ST elevation Myocardial Infarction (Major Heart Attack) landed in our emergency with no financial capacity or responsible bystanders. They did not even have money to do the registration.

Dr.Sambath and Emergency Department staff did a wonderful job in stabilising him.
Dr. Prabhakaran did the initial work up, coordinated with multiple phone calls to patient’s friends and family and mobilising them.

This gentleman had sustained a massive MI involving the proximal Left Anterior Descending artery as indicated by his ECG.
He is already in the verge of crashing, going for frank pulmonary oedema and cardiogenic shock if proper revascularisation is not done urgently.
No money, no responsibile bystanders and no time to waste. It’s an “ ethical dilemma “ !

Our Branch head Mr. Rajesh gave us the moral support and there’s no other option but to proceed.
We shifted him to Cathlab in record time and primary angioplasty was performed, opening up a totally occluded LAD. Patient is well and stable now.

My heartfelt gratitude to our directors and management for their unwavering support which gives us the confidence to provide life saving care in an emergency even in these difficult situations🙏🏻

Today we had a 29 year old young man for coronary angiogram. He had chest discomfort two weeks ago and was treated elsewhere and referred to us. Apart from mild chest discomfort that day he was remarkably asymptomatic. He has no identified risk factors- no family history, not a diabetic, no high blood pressure , cholesterol is not that high, non smoker. I was expecting normal coronaries or mild disease. But what we saw in the angiogram was shocking! 100% occluded LAD, one of the main branches of Left Coronary artery and 60% narrow Right Coronary artery!
He inderwent a successful procedure with excellent results but why he developed it at this age?
Being South Asian descent is a risk factor.

Life is a precious gift and we often take it for granted until a moment comes when we fear of losing it.

This gentleman who leads a hectic lifestyle managing a company was fine that evening, two weeks ago till he felt something like an indigestion. It was not getting better so he drove to the hospital with his wife. My first on call Dr. Binoy saw him in the emergency with ECG changes of an acute heart attack. He was immediately shifted to the CCU with plans to start early treatment so that damage to the heart is minimal.

But then he arrested. During a heart attack when the heart muscle ceases to get blood supply sometimes it doesn't know what to do. The electrical activity that keeps our heart beat goes haywire and the heart stops beating but just quivers. This is called ventricular fibrillation and often occurs during the early phase of a heart attack and cause death before people reach hospital. Only thing to save life here is early CPR and defibrillation.

Defibrillator is a device that is capable of giving high energy shocks that can jumpstart the heart again to beat properly. As this gentleman arrested inside the hospital he received immediate Defibrillation which restored his heart beat but only temporarily. He arrested recurrently and had to be shocked eight times. His blood pressure started dropping and he started having early pulmonary oedema.

Pulmonary oedema is a condition where fluid seeps into the lung and makes breathing difficult and causes oxygen in the blood to drop. This often happens when the heart starts failing to pump enough blood.

Only chance for the patient's survival now is early opening up of the blocked coronary artery by PCI ( per cutaneous coronary intervention). The Catheterisation lab is already ready and the PCI team is ready to take over. By the time I arrived Dr. Manoj the anaesthesiologist on call is already taking care of the patient's breathing problem. But his blood pressure still remains 70 systolic despite two drugs on flow to keep it up.
By the time we finished the procedure and opened up the clogged artery it was almost 2 am. But to see his smile now makes all the nightmare we endured that night worthwhile
