NOT ALL HEART ATTACK PATIENTS REQUIRE BYPASS
It has come to my attention that certain heath institutions are doing heart BYPASS Surgery on all patients who have had previous heart attacks.
This practice is not right and in a sense negligent. Heart attacks results in deaths of heart muscles if not successfully opened up with drugs or stents. Some muscles remain alive but inactive as a protective mechanism by reducing metabolism.
A patient who has had a big attack must undergo a special radioactive heart test called SESTAMIBI where radioactive potassium is injected into the veins and allowed to illuminate the substance giving areas of death muscles and those alive. If alive or what we call viable muscles make up > 20% then BYPASS is recommended but if

1. Echocardiogram - trans thoracic Echo or heart scanner
2. Doppler - study of blood vessels and blood flow (use in stroke medicine and many other conditions
3. General Scanning (Obs & Gynae, MSK, breast , joints etc)
4. Stress echocardiography - more sensitive and specific than exercise stress test to diagnose, risk stratify and prognosticate Coronary artery diseases.
We will keep our valued clients updated on the machine arrival. By the way, we have expertise on the use of this machine. Obs & Gynae scan will be provided by qualified and experienced sonographer.
The Current issue of an infant with a rare mitochondrial condition with stroke in a London Hospital with parents urging to try a new treatment in the US whilst the British doctors advise otherwise is a case in point of the Capacity Act 2007.
The child has MELAS - Mitochondrial Encephalopathy with Lactic Acdosis & Stroke.
The infant does not have the Capacity to decide what's best for herself. In this case, the Act does give the legal right to the doctors to make the decision in the patient's best interest. That's what the doctors have made. The Act does not give the parents the right to decide for their child.

The Capacity Act 2007 stipulates that All British Citizens have the right to; right information regarding their ailments, the right to independent assessment and decision, the right to be partners in the decision making process of their health.
The Act accepts a Capacitated patient to be;
1. Cognitively sound
2. Able to assimilate appropriate treatment options given
3. Able to assess the benefit risk ratio
4. Make an informed decision
5. Inform the healthcare worker their decision
Regardless of the opinion of the healthcare worker, the patient decision supersedes the direction of management.
Patient in this Act have been allowed to be partners in the decision making process of their own health rather than someone taking sole decision making process of the patient health and life.
