ANNOUNCEMENT TO ALL CMCians!!!!

Please be reminded of the Clinic Requirements to be completed during the signing of clearance:

-Submit a 2x2 ID Photo.
-Update your Directory Information.
-Fill out the form through the Google Forms link provided by your respective mayor.

Make sure all information is accurate and complete before submitting.

Your cooperation is essential to ensure a smooth and organized clearance process.

Important: Health Profiling for Updates Requirement for Clearance Signing

As part of the requirements for signing your clearance, you are required to complete your health profiling for updates. This is a mandatory step that must be completed before your clearance can be signed.

We have assigned medical volunteers in each section.

Important: Recitation of Prevent Teenage Pregnancy Campaign

As part of our ongoing efforts to promote awareness and prevention of teenage pregnancy, we have assigned specific CMC medical volunteers to each section. Please look for your respective volunteers and recite the campaign message with a heart!

Section Assignments

- Section A: Enoc - Kyle
- Section B: Sheina LaLa - Ian Duyugan
- Section C: Janice Dag-um - Ammy Alcordo
- Section D: Krisalyn Cajes - Rufa Mae Castor
- Section E: Princess Litub - Jacqueline Espina

Recitation Instructions

1. Please meet with your assigned volunteer and review the campaign message.
2. Recite the message with conviction and passion.
3. Show your commitment to preventing teenage pregnancy by speaking from the heart.

Remember:
Every voice counts, and every recitation can make a difference in promoting awareness and prevention of teenage pregnancy.
