A 58-year-old hypertensive male presents with acute onset severe epigastric pain for 2 days, associated with vomiting, abdominal distension, obstipation, and decreased urine output. On examination, he is tachycardic with generalized abdominal tenderness and guarding. Bowel sounds are sluggish.
