It has come to my attention that an individual has been using my name and license number to issue falsified medical certificates using an e-prescription from my previous hospital affiliation.

If you, your organization, hospital or clinic has received any of these documents that appears fraudulent, kindly reach out to verify its authenticity.

due to this, I am no longer issuing e-prescriptions and medical certificates via tele-consultations.
