Good day, LCCians!

The Center for Guidance and Counseling will officially begin signing your Clearance Sheets tomorrow, May 9, 2024. With that, below is the list of Clearance Requirements for each Grade/Year Level:

** Kinder to Grade 9 Students:

1. Complete Cumulative Record Form
2. Informed Assent Form
3. Guidance Services Evaluation Form

** Grade 10 Students:

1. Complete Cumulative Record Form
2. Informed Assent Form
3. Guidance Services Evaluation Form
4. Exit Interview Form (Proceed to the CGC)

** Grade 11 Students:

1. Complete Cumulative Record Form
2. Informed Assent Form
3. Guidance Services Evaluation Form

** Grade 12 Students:

1. Complete Cumulative Record Form
2. Informed Assent Form
3. Career Talk Evaluation Form
4. Guidance Services Evaluation Form
5. Exit Interview Form (Proceed to the CGC)

** 1st Year College Students:

1. Guidance Services Orientation Evaluation Form
2. Complete Cumulative Record Form
3. Informed Assent Form
4. Guidance Services Evaluation Form

** 2nd Year College Students:

1. Complete Cumulative Record Form
2. Informed Assent Form
3. Guidance Services Evaluation Form

** 3rd Year College Students

1. Complete Cumulative Record Form
