Qualifications: RA 1080 (Registered Social Worker)

Requirements (to be accomplished in three sets)
1. Fully accomplished and notarized PDS (CSC Form 212, 2025)
2. Application letter
3. PRC ID/Board Rating, and other relevant certificates.

Thank you Mayor Alfred Romualdez and Philhealth for the support you have extended to TCH for processing of accreditation as GAMOT Facility and recently accredited Out Patient Mental Health... Philhealth reports as well to the yMayor through the Office of RVP Jubay the increase of Philhealth reimbursements of TCH Animal Bite Treatment Center when it started to increase services of animal bite from Monday to Saturday from previous Monday and Thursday...
