1. DULY- SIGNED APPLICATION LETTER indicating the POSITION TITLE addressed to Dr. RONNIE GREGORIO B. GIGANTONE III, Medical Center Chief II.
2. Fully accomplished and NOTARIZED PERSONAL DATA SHEET (PDS) with recent passport-sized picture (ANNEX H-1 - CS Form No. 212, Revised 2025) and with WORK EXPERIENCE SHEET (ANNEX H-2 - CS Form No. 212 Attachment) which can be downloaded at csc.gov.ph (NOTE: if currently employed, Work Experience Sheet must be countersigned by the supervisor)
Note: Kindly use the revised form of the Personal Data Sheet (PDS)
3. PERFORMANCE RATING in the last rating period.
4. CERTIFICATE OF CSC ELIGIBILITY/ PRC LICENSE.
5. CERTIFICATE OF TRAININGS/ SEMINARS relevant to the position being applied to, undertaken within the last five (5) years.
6. CERTIFICATE OF EMPLOYMENT/ SERVICE RECORD with summary of actual duties relevant to the position being applied to.
7. TRANSCRIPT OF RECORD AND DIPLOMA
