During an air transfer, the cabin is pressurized most of the time although some aircrafts specially helicopters are unpressurized — but not exactly like sea level. It’s more like being on a mountain, where there’s a little less oxygen in each breath. For most people, that’s fine. For some patients, it needs planning.

Here’s what families should know:

✅ The SpO₂ number on the monitor shows how much oxygen is in the blood. The crew and doctor watch this closely.
✅ The flight doctor decides if extra oxygen or a special flight profile is needed.
✅ Our team plans the route, altitude, and onboard oxygen around that medical plan. We also consider what aircraft to use if there is a need for a sea level flight.

Before the flight, we can brief your family liaison so you know what to expect.

For patients with disabilities, a transfer isn’t “just another trip” — it’s a move that has to respect mobility, communication, and support needs.

When we coordinate an air transfer, we consider:

✅ Mobility aids – How wheelchairs or other supports will be moved and secured from ground to cabin and back to ground.
✅ Hearing/visual comms – Using clear, simple briefings and the patient’s preferred way to receive information.
✅ Caregiver seating – Planning space for a parent, guardian, or caregiver to stay near whenever possible.

Behind every medical flight is a human crew with strict duty time rules.

Before we say yes to a launch, we check:
✅ How long the pilots and medical crew have already been on duty
✅ Required rest time between missions
✅ Night operations limits and workload
✅ Weather and runway conditions for that window

If the risk of fatigue is too high, we move the flight to a safer time instead of pushing through.

Some patients need a ventilator even while they’re being moved between hospitals.

During a ventilated transfer, our team works with the sending doctors to make sure:
✅️Patient needs: The specific type of transport ventilator used depends on the patient's condition and the needs so medical team should bring the right type of ventilation system

✅️Patient transport equipment: Some aircraft have dedicated patient transport compartments with access to high-pressure oxygen and power for medical equipment. Always bring extra oxygen tanks since you still need portable tanks when moving the patient in and out of the aircraft.

✅️Crew and patient separation: For flights with potential for high viral transmission, a more elaborate ventilation system may be required, ensuring the patient and crew have separate air supplies.

✅️The receiving transport ground ambulance and hospital knows the type of vent system used and its settings

We confirm that the receiving hospital is ready:
✅ Information Exchange: Timely and accurate sharing of a patient's medical history, current condition, test results, medications, and the reason for transfer is essential. This is often facilitated through phone calls, secure messaging, or electronic health record (EHR) systems.

✅ Role Clarity and Accountability: All participants, including referring and receiving teams, need a clear understanding of their roles and responsibilities during and after the transition of care.

✅ Proactive Planning: The receiving hospital uses the information provided to prepare the necessary resources, equipment, and a medical team before the patient's arrival, especially in the case of critically ill patients or mass casualty events.

✅ Patient and Family Engagement: Patients and their families are involved in the process, informed of the care plan, and educated on what to expect during and after the transfer.

✅ Resource Management: In a regional or system-wide context, coordination centers may be used to balance hospital loads and ensure equitable access to care by directing patients to the most appropriate available facility.

✅ Use of Protocols and Tools: Standardized protocols, such as the ISBAR (Identify, Situation, Background, Assessment, and Recommendation) tool for handovers, help ensure all relevant information is communicated effectively and consistently.
