Action taken: patient was transported for medical intervention at RHU HEMS. oxygen saturation taken(within normal)
Medication given (IM) by Mrs.Janette Cabeltes(Birthing Facility Staff).

Details of incident: HEMS received a call from MDRRMO Pilar that there is a patient in need of medical assistance, suffering from Difficulty of breathing. Mr. Ken Cantonjos, staff on duty immediately went to the said location. Upon arrival, scene was crowded and people gathered around a man who was pacing and shouting violently. The patient was later convinced to be transported to HEMS for assessment and the proper medication.

Injuries/NOI: Numbness (lower extremities) both legs. Laceration on the back of the head.

Action taken:assist patient carefully in the stretcher while the head and neck was supported firmly. Initial assessment was done, vitals are taken and recorded. Coordinated and referred patient to higher facility.

Details of incident: Staff on duty Mr. Ken Cantonjos received a call from MDRRMO Pilar that there has been a motor boat coming from a nearby coastal area carrying a patient in need of medical assistance. After the vessel had docked at Pilar port, the patient was then identified to have a sustained head injuries after being slipped on the floor (as verbalized by the patient) due to numbness of both legs, resulting in a lacerated wound on the back of the head. Referred to Donsol District hospital for further evaluation and management.

Injuries/NOC : DOB few hours PTA

Action taken: Patient was put on fowlers position and placed on a stretcher to assist breathing. Hook to oxygen at 6 LPM via nasal cannula. Vitals was taken and recorded.Coordinated to DDH via messenger prior to departure.

Details of incident: An elderly patient arrived at Hems, accompanied by his daughter, patient is obviously suffering from oxygen deprivation, with a known condition of asthma disease. The patient's vitals are unstable with low oxygen saturation and Initial assessment was immediately done by Mr. Ken Cantojos, staff on duty. He was placed on stretcher and transferred to ambulance. Oxygen therapy was administered via nasal cannula. The patient was transported to Donsol District Hospital and endorsed to the nurse on duty for further evaluation and management.

Details of incident: staff on duty Ms. Lorie Ibañez received a call from brgy Calongay for Medical Assistance and transportation of a bed ridden patient with IC, picked up, and coordinated to District Hospital. Referred at ER for further evaluation and management, hence NOD of DDH referred the patient to Higher Facility for further management. Patient assisted by Mr.Losamia from DDH to MetroHealth Hospital.
