Magandang Araw po.
Ipinababatid po namin sa lahat ng nangangailangan ng dialysis na ang Pulilan Hemodialysis Center, na matatagpuan sa TESDA Building, Bukid Government Center, Peñabatan, Pulilan, Bulacan, ay bukas na at tumatanggap na ng mga pasyente.
Ang aming pasilidad ay handang maglingkod para sa:
* mga bagong pasyente, at
* mga pasyenteng nagnanais lumipat ng dialysis center.
Layunin po ng Pulilan Hemodialysis Center na makapagbigay ng ligtas, de-kalidad, at ramdam na serbisyong pangkalusugan para sa kapakanan ng bawat pasyente.
Mga Kinakailangang Requirements para sa Dialysis:
MEDICAL:
1. MEDICAL ABSTRACT / HEMODIALYSIS PRESCRIPTION
2. LAST 3 HEMODIALYSIS TREATMENT SHEETS
3. UPDATED LIST OF MEDICATIONS
4. HEPATITIS PROFILE (HBsAG, anti HCV, anti-HBs)
* Results should not be more than 3 Months
* HBsAG and anti-HCV should be NON-REACTIVE
5. LABORATORIES (CBC, BLOOD CHEMISTRY)
* Results should not be more than 1 Month
* No Hemoglobin result of below 8g/dL
6. RT-PCR / COVID SWAB TEST RESULTS
* Results should not be more than 1 week
* Should be NEGATIVE results
7. CHEST X-RAY
* Not more than 1 Month
* No current lung infection
BILLING:
8. CERTIFICATE OF PHILHEALTH UTILIZATION (from other hospitals / centers)
9. PHILHEALTH MEMBER DATA RECORD / PHILHEALTH ID
10. PHILHEALTH DIALYSIS DATABASE REGISTRATION (PDD) / CONFIRMATION LETTER
11. CSF SIGNED BY MEMBER / & EMPLOYER (if employed)
12. PROOF OF PAYMENT OF PHILHEALTH CONTRIBUTION
Any of the following:
- Certificate of contribution issued by the employer (if employed)
- Copy of OR of Philhealth Contributions (if voluntary member)
- Certificate of Sponsored Membership (if sponsored member)
13. PWD / Senior ID
