Asian rhinoplasty has become a burgeoning segment of nasal surgery owing to the explosive popularity and expansion of cosmetic surgical procedures in the Asian population, both in the Western and Eastern hemispheres. As it has grown into one of the most popular facial plastic procedures for Asian patients, the surgical approaches and techniques employed during Asian rhinoplasty have also developed an increasing level of sophistication and complexity, while producing improving aesthetic results in a more predictable and durable manner.

The salient aspects of Asian rhinoplasty include not simply the individual nature of the anatomy, but the very personalized aesthetic unique to each patient. Rhinoplasty, whether reductive or augmentative, is not an algorithmic operation with a rote sequence of steps which, once executed, results in a functionally and cosmetically successful operation. A beautiful nose, based on canonical measurements of angles and distances, may, in the context of the entire face result in disharmony, and a less than optimal result.

An appreciation for the concept of Asian beauty is essential to achieving a result during Asian rhinoplasty that will not only create a beautiful nose, but a nose that will enhance a patient’s overall appearance. Asian patients are not seeking to appear “Westernized”, an antediluvian notion some misguided surgeons have erroneously propagated, but rather to achieve characteristics and features that more closely resemble others within their ethnicity. Most nose surgery surgeries are approached in a similar fashion; they all begin with incisions, either in the closed or open style. Regardless of where your incisions are placed, cartilage and/or bone will be removed to lessen the size of your nose.

The nasal septum may be deviated, which requires correction to improve both the appearance of the nose and its function. Nasal obstruction may also be caused by a collapse of the tip cartilages resulting in external valve collapse. Deviation of the nasal septum along with deficiencies in upper lateral cartilages are a common occurrence in revision cases resulting in internal valve collapse requiring the use of cartilage grafts called "spreader grafts" to spread open this narrowed area.

In many of these cases, given that there has been an important damage in the structure of the nose in prior surgeries, it is necessary to reconstruct what was lost or damaged.

Some surgeons use the patients' own tissues, or synthetic tissues, or a combination of both. If the patient's own tissues are used, they can be rib cartilage, septum cartilage or ear cartilage.

Personally, we prefer (in a case that needs a reconstruction of the support and structure of the nose) to use ear cartilage. It is strong, flexible and malleable enough to recreate most of the cartilaginous nasal structures (which are the ones that are most damaged by prior surgeries).

The practical difference between open rhinoplasty versus closed rhinoplasty is small, but obtaining the best results for the patient is important.

The closed technique allows for faster operation and less swelling during the recovery period.

On the other hand, an open rhinoplasty surgery allows for more precision for difficult noses and when altering the nasal structures for more complex cases like revision rhinoplasty.

Revision rhinoplasty often requires cartilage for structural support for the tip and/or bridge of the nose. Septal cartilage is often deficient and plastic surgeons often must choose cartilage from elsewhere in the body, particularly the ear and occasionally the rib.

Soft tissue irregularities along the bridge of the nose or tip may also require tissue that provides a cover or camouflage and acts as a "natural filler" which is permanent. An excellent source is "temporalis fascia," which can be taken from a remote incision within the hairline and results in no noticeable deficit. Occasionally, scarring within the nose requires the use of skin grafts to reopen a distorted and narrowed nasal airway.

In many of these cases, given that there has been an important damage in the structure of the nose in prior surgeries, it is necessary to reconstruct what was lost or damaged.

Some surgeons use the patients' own tissues, or synthetic tissues, or a combination of both. If the patient's own tissues are used, they can be rib cartilage, septum cartilage or ear cartilage.

Our personal experience (in a case needs a reconstruction of the support and structure of the nose) is to use ear cartilage. It is strong, flexible and malleable enough to recreate most of the cartilaginous nasal structures (which are the ones that are most damaged by prior surgeries).

With a small amount of cartilage extracted from the ear, we can solve almost all the cases of nasal reconstruction derived from problems caused in prior surgeries.

It is usual for surgeons to use aggressive techniques, to remove a lot of cartilage to narrow the nose or make it pointier. This has negative aesthetic and functional consequences in long term, meaning that the nose will look bad and, to top it off, the patient will not be able to breathe well! ).

The auricular cartilage, especially from the auricular shell, is ideal to reconstruct the entire nasal tip, giving it an esthetically adequate shape again. At the same time it solves breathing problems and can solve profile defects, for example in the cases where a big nasal hump is removed. We can even also close septum perforations.
