NATIONAL COVID-19 VACCINATION DAYS
November 29 to December 1 💉

Sang-ayon sa Proclamation No. 1253, magasasagawa ang Lungsod ng Santa Rosa ng malawakang pagbabakuna sa mga araw na ito. Ito ay gaganapin sa mga nakatakdang vaccination sites ng lungsod.

Ang mga manggagawa at empleyado ay hindi magiging absent kung sila ay magpapakita ng katibayan na sila ay nabakunahan sa mga araw na ito.

🤔 SINO ANG BABAKUNAHAN?

✅ Adult Population o may edad 18 pataas
✅Pediatric Populaiton o may edad 12 hanggang 17

🤔 ANO ANG DAPAT DALHIN UPANG MABAKUNAHAN?

Adult Population
✅ Valid ID or Proof of Residency
✅ Medical Certificate/Clearance kung mayroong comorbidity

Pediatric Population
✅ Kailangan kasama ang magulang o guardian
✅ Valid ID ng minor at Valid ID ng magulang o guardian
✅ Birth Certificate o anumang patunay ng relasyon sa minot sa kasamang magulang o guardian
•Medical Certificate/Clearance kung mayroong comorbidity ang minor

🤔 PWEDE BA ANG WALK-IN?

✅ Maaaring mag walk-in kapwa ang registered at hindi registered sa Santa Rosa Vaccination Portal
✅ Hinihikayat ang lahat na mag-rehistro sa Vaccination Portal upang mapabilis ang proseso ng verification at screening sa araw ng pagbabakuna
✅ Para sa mga hindi registered, magpa-rehistro sa mga on-site registration area sa mga nakatakdang vaccination site sa mga nasabing araw

🚍 May Libreng Sakay ang Pamahalaang Lungsod para sa National Vaccination Days. Makipag-ugnayan sa inyong mga barangay.

👀 PAALALA:

👍 FIRST DOSE ang ibibigay sa mga araw na ito upang mabakunahan ang mga hindi pa nakatatanggap
👍Maaaring pumunta ang mga naka-schedule para sa SECOND DOSE
👎Hindi pa magbibigay ng BOOSTER shots sa mga vaccination sites

This is the official page of School Clinic of Labas Senior High School.
Regular office hours are from 8:00 am to 5:00 pm, from Monday to Friday.
The programs and services are open to all LSHS students only who are currently enrolled to our school.

The main objectives of this service is the prevention of illness as well as the promotion of health and well being of the students through:
a. Development of healthy attitudes and healthy behaviors by students
b. Health Education and Healthy Tips
c. Early detection and care of students with health problems
d. referral system and coordination with teachers
