Each specialty undergoes years of focused training to deeply understand specific systems of the body. Obesity is not just about weight — it involves hormones, metabolism, insulin resistance, and long-term systemic monitoring. PCOS is not just acne or irregular periods — it involves reproductive health, endocrine balance, and fertility considerations.

There are several reasons why I remain cautious in prescribing GLP-1 receptor agonists such as semaglutide or tirzepatide.

While these medications have shown promising results, they are still relatively new compared to many long-established treatments. With newer drugs, long-term safety data — especially beyond several years of widespread use — may still be evolving. As a clinician, I tend to be conservative when it comes to newer injectable therapies, particularly when they are being used beyond their primary indications.

I have also heard anecdotal reports and off-label observations from colleagues — including endocrinologists and internal medicine specialists — regarding certain effects that are not yet fully consolidated in long-term data. While these observations are not definitive evidence, they are enough for me to practice thoughtful caution.

Medicine requires both openness to innovation and respect for long-term safety. For now, my approach is to observe ongoing research and outcomes before personally initiating or widely recommending these medications.

This is simply my professional perspective. I strongly encourage anyone considering semaglutide or tirzepatide to consult directly with their endocrinologist or internal medicine specialist to determine what is most appropriate for their individual case.
