Physician Forum by Department of Internal Medicine Saleem Memorial Hospital Today.

A case of Ocular Toxoplasmosis in a 22 year old male was presented by Hunza Malik. The patient developed drug induced pancytopenia due to pyrimethamine. He was managed with Platelet Megaunit transfusion, Leucovorin and Eltrombopeg. His vision improved. Dr. Sidra Riaz(Opthalmologist) and Dr. Bilal (Rheumatologist) participated via Zoom.

Fortnightly Physician forum meeting at Saleem Memorial Hospital Lahore today.

Cardiology Team from SMH led by Dr. Masood Ahmed presented a case of 27 year old male who has been having non-specific malaise, lethargy, and feverish feelings for the past few months.
He now has difficulty using his right arm, having fatigue numbness and pain.
He has also been diagnosed with hypertension, which is uncontrolled with treatment.

His BP was 170/110. All pulses in right upper limb were impalpable.

ESR was 95. RFTs, LFTs and Electrolytes were all normal

A Diagnoses of Takayasu Arteritis was made.

Aortogram revealed markedly stenotic right subclavian, left internal carotid and Right Renal arteries.

Successful Stenting to Right subclavian and right renal artery resulted in relief of pain in right arm and cure of hypertension.

He was referred to Rheumatologist/ Immunologist for management of Vasculitis.

Intrventional neurologist ruled out stunting to left carotid as it has fibrosed beyond correction and left hemisphere is having good collateral through circle of Willis.
