22 yearof age Unmarried girl presented with pv bleeding associated with lower abdominal pain mild to moderate for last 1 month...pt took multiple antibiotics and oral progesterone for pv bleeding from private clinics...O/E tenderness on right iliac fossa ,vitally stable.ultrasound showed 5*5 cm complex ovarian cyst...CA 125 was 24..admitted for laparoscopy and proceed after consent and optimization for surgery with safety labs done.
Intra operative findings:
5*5 cm Right Turbo ovarian mass suspicious for chronic ectopic with leak 100 ml peritoneal hemorrhagic fluid...Right salpingectomy done tissue attached with right o***y removed...Left tube and o***y and uterus normal...(.at the spot UPT done result was positive for pregnancy)
Drain placed abdomen closed in reverse order.
Tissue sent for histopathology..
Pitfalls:::
pt differentials were suggestive of miscarriage or ectopic,molar pregnancy and B HCG as a protocol should be advised early..

Case of P0A0 Multiple fibroids with Venous thromboembolism phenomena

P0A0 42 years old Married for 20 years with heavy menstrual bleeding for 5 years had history of previously midline scar operated for ovarian masses 12 years ago, plus history of abdominoplasty n hernia repair with mesh 8 years ago..9 months back her Above knee amputation done due to VTE phenomena ..now on ultrasound huge multiple fibroids largest one measuring 10 ×8 cm on P/A pelvic mass upto epigastrium , irregular immobile firm lower limit not reachable P/V examination v/v healthy uterus upto 20 wks size mobility restricted cervix normal fornices clear no pv bleeding or discharge..after optimising for surgery High risk consent for TAH taken as pt doesn't need fertility n wants definitive treatment

Total hysterectomy done with midline incision n repair of mesh .

OVARIAN CYST N primary infertility

P0A0 with primary subfertility 28 years of age,BMI 30 kg/m² married for 5 years, Her Last menstrual period date 25 March 2024,5/28 day regular cycle with normal flow, presented with complaint of inability to conceive and vague abdominal pain on ultrasound 7×7cm right sided cyst aspirate after local anesthesia with16 G spinal needle 140 ml fluid aspirated 20 ml sent for cytology, patient was hemodynamically stable at the end of procedure called for follow up after 4 weeks.

Case of OVARIAN CANCER

58 years P0A1 postmenopausal for 10 years( surgical menopause) presented with complaint of abdominal swelling for 1 year associated with early satiety bloating
and shortness of breath on examination A mass 15 ×15 cm arising from right iliac fossa mobile firm to cystic , nontender normal temperature overlying skin is normal and lower limit is reachable, on per speculum vaginal vault healthy looking no bleeding or discharge .On vaginal examination no uterus felt firm to cystic bilateral masses about 20 wks size were palpated.
On ultrasound bilateral ovarian masses right 10 cm with septa and multiple solid component measuring 19mm to 32mm , on colour dappled blood flow is present in solid component

Left sided cyst 15 cm with solid element 54mm
N blood flow+ve
No free fluid seen in pelvis

Her CA 125 427U/ml
Making her
"RISK OF MALIGNANCY INDEX"

Ultrasound findings(3)×CA 125 (427)×menopausal(3) = 3843

Pt optimize for surgery
Staging laparotomy done at Riaz hospital faisalabad
Midline incision
Peritoneal washing taken
Bilateral ovarian masses Left 13 ×13 cm mass with intact capsule n no tumor on external surface
Right 8 × 8 cm complex ovarian mass with tumor on external surface which was adherent to lateral pelvic wall n sigmoid colon.
Infracolic omentectomy done
Diaphragm, under surface of liver palpated
Tissues sent for histopathology n pt follow up with chemotherapy plus post op thromboprophylaxis
Counselling about lifelong follow up.

Case of Primary subfertility due to Multiple fibroids

P0A0 ,28 years of age Married for 10 years came with complaint of inability to conceive and lower abdominal heaviness and abdominal swelling for 8 years associated with heavy and painful menstruation soaking 4-5 pads / day ,cycle 7 /28 day ,regular, h/o painful coitus on examination pale
and spoon shaped nails, abdomen soft nontender 10 ×10 cm size mass irregular firm mobile occupying subumblical region lower limit not reachable, on per vaginal , bimanual examination uterus 16 to 18 wks size irregularly enlarged, mobile, nontender,both adnexa clear, on per speculum examination cervix healthy looking with no bleeding or discharge pap smear taken.
Her ultrasound report showed multiple fibroids largest one 7×7 cm on anterior wall , not distorting endometrial cavity.
Patient optimize for surgery and her multiple myomectomies done , endometrial cavity not breached...tubal patency with methylene blue showed bilateral patent tubes with free spillage ovaries were normal n healthy looking..
Pt was called for follow up after 10 days.
And 6 months contraception advised.

Case of Endometrium Cancer
Stage 2
Summary

54 years old P5A0 , diabetic for 10 years on insulin,menopausal for 8 years presented with postmenopausal bleeding and pv discharge for 2 months n significant wt loss 7 to 8 kg, shortness of breath n early fatiguability,
TVUSS showed endometrium 5.8 mm and heterogenous mass invading into myometrium.
On MRI report7×7×6 cm mass in endometrium infiltrating right lateral wall of uterus upto >50% of its depth , no parametrial invasion seen, lobulated. Component extends upto cervical canal upto the level of internal os , no definite invasion of walls of cervix.no associated suspicious pelvic lymphadenopathy
HB 8.8
ANTI HCV +ve
Endometrial biopsy advised
Patient optimize for surgery
