# (ADHD) is one of the most common behavioral conditions seen in children, yet it is often misunderstood or overlooked. Many parents initially assume their child is simply “energetic” or “easily distracted,” but persistent symptoms may indicate something more.

Children with ADHD may struggle to focus, sit still, or control impulses, which can affect their academic performance and social relationships. Importantly, these behaviors are not due to poor parenting or lack of discipline—they are linked to how the child’s brain processes attention and control.

Early recognition is crucial. If symptoms appear both at home and school and continue for several months, a professional evaluation is recommended. The good news is that with early support, structured routines, and appropriate interventions, children with ADHD can thrive academically and socially.

As parents, your role is not to “fix” your child, but to understand them, support them, and advocate for the help they need. Awareness is the first and most powerful step.

🚑 EMERGENCY MANAGEMENT OF PEDIATRIC MENINGOCOCCEMIA
Definition
Meningococcemia is a rapidly progressive bloodstream infection caused by Neisseria meningitidis that can lead to septic shock, disseminated intravascular coagulation (DIC), and multi-organ failure.
It represents a pediatric medical emergency requiring immediate recognition and treatment.
⚠️ Clinical Presentation
Common early findings:
• High fever
• Lethargy or irritability
• Vomiting
• Tachycardia
• Hypotension
Characteristic finding:
Petechial or purpuric non-blanching rash
Severe disease may progress to:
• Septic shock
• Disseminated intravascular coagulation (DIC)
• Waterhouse-Friderichsen syndrome
• Multi-organ failure
🚨 Initial Emergency Assessment (ABCDE)
Airway
• Ensure airway patency
• Early intubation if decreased consciousness
Breathing
• Provide high-flow oxygen • Monitor oxygen saturation continuously
Circulation
• Establish IV or intraosseous access • Continuous cardiac monitoring
💧 Fluid Resuscitation
First-line treatment for pediatric septic shock.
• 20 mL/kg isotonic crystalloid bolus
Repeat boluses up to:
40–60 mL/kg during the first hour
Monitor:
• Capillary refill
• Blood pressure
• Urine output
• Lactate
💊 Empiric Antibiotic Therapy
Antibiotics must be given within the first hour of suspected sepsis.
Recommended therapy:
Ceftriaxone 50–100 mg/kg/day IV
OR
Cefotaxime 150–200 mg/kg/day IV divided doses
If meningitis suspected → continue full meningitis regimen.
🩸 Vasopressor Support
Indicated when hypotension persists after fluid resuscitation.
First-line agent:
Epinephrine infusion 0.05–0.3 mcg/kg/min
Alternative:
Norepinephrine infusion
⚠️ Management of Complications
Disseminated Intravascular Coagulation
• Platelets
• Fresh frozen plasma
• Cryoprecipitate
Adrenal Hemorrhage
(Waterhouse-Friderichsen syndrome)
• Stress-dose hydrocortisone
Respiratory Failure
• Mechanical ventilation
🔬 Diagnostic Workup
• Blood cultures (before antibiotics if possible)
• CBC with differential
• Coagulation profile
• Serum lactate
• Electrolytes and renal function
• Lumbar puncture (if stable)

Complications:
• Orchitis (in males)
• Oophoritis (in females)
• Meningitis
• Pancreatitis
• Hearing loss (rare)

✅ What is Crigler massage ?
Crigler massage is a simple lacrimal sac massage used in infants and young children to treat congenital nasolacrimal duct obstruction (CNLDO)—commonly called a blocked tear duct.
It works by creating hydrostatic pressure in the lacrimal sac to help open the membranous blockage at the lower end of the nasolacrimal duct.
