Large abdominal mass of pre peritoneal origin in a 75 year old lady which turned out to be "lipoleiomyoma". The mass weighed 5.1 kg and measured 29cmx20cmx13cm. Alhamdolillah it was well encapsulated to be removed easily. Patient recovered smoothly.

Peritoneal carcinomatosis in a young patient.
Persistent and recurring Nonspecific abdominal pain needs further investigation with CT scan and diagnostic laparoscopy. In this case CT scan missed these small metastatic depostis in peritoneum.

Young male with significant abdominal symptoms and weight loss of 20kgs over 6 months, Right sided colonic tumor on colonoscopy, biopsy non conclusive, CT showed circumferential mass in caecum and ascending colon and large lymph nodes in mesentry, referred for laparoscopic biopsy, planned to do right hemicolectomy as mass was causing subacute obstruction. During Right hemicolectomy found 3 discrete masses in terminal ileum, caecum (marked in pic 1) and ascending colon (not visible in pic 1) and very large lymph nodes. Rest of bowel was normal, no other lesions or skip areas. Right hemi with stapled ileo colic anastomosis done, which leaked on day 5. Surprisingly Perforation was just after anastomosis in transverse colon. Repaired leak and this time made diversion ileostomy proximal to stoma, suspecting crohns. Patient recovered well. Histopathology turned out to be intestinal TB. Put him on ATT for 2 months, had a very good response om ATT alhamdolillah, gained 10 kgs weight in 2 months. Reversal done last week, it did not leak this time.
After thoughts, may be it was a good option to do lap lymph node biopsy the first time. Thoughts??
