Dear junior DPTs!

I want to encourage you to consider starting your own practice. Jobs may be hard to find, and salaries aren't always great for fresh graduates. With just a year of experience, running your own practice can teach you a lot. Keep learning!

As physical therapists, we need to think creatively and raise awareness about what we do. Be active on social media, Learn about digital marketing, social media Ads, SEO, podcast, videos, graphic designing etc, Promote your practice online, it can help you succeed.

Don't worry if you're a middle class physical therapist, You don’t need expensive equipment to start a private practice. A space and some resistance bands are enough. Let’s support our profession together! Instead of blaming our noble profession on social media, work for the profession

Many DPT graduates are struggling to find jobs, and a big part of the problem lies with their institutes and teachers. Most institutions don’t offer clear guidance for graduates on what steps to take after finishing their studies. Pofessors often don’t provide support on how to start their own practices or manage a clinic. When the focus shifts to making money instead of helping students succeed, the result is a lack of job opportunities for these new professionals. It’s important for educational institutions to do better in preparing graduates for the real world.

I remember an old school PT technician at the hospital where we worked who considered himself the best Physiotherapist. He once said, "The more you stretch patients aggressively, the more satisfied they will be." 😀

So, please be more of a therapist and less physical.

Part (1) How to Spot the Difference: COMPETENT VS ORDINARY PHYSICAL THERAPIST!

ORDINARY PHYSICAL THERAPIST:

1) An ordinary physical therapist relies heavily on passive treatments, electrotherapy, as their primary approach. Remember, passive therapies are only part of the bigger picture.

2) An ordinary physical therapist spends excessive time on passive treatments.

3) An ordinary physical therapist uses ineffective and misleading terms like "poor posture," "low back pain due to a weak core," "your spine is misaligned," or "your pelvis is out of place."

4) An ordinary physical therapist instills fear of movement by saying things like "You can’t hike or run," "You need to stop weightlifting," or "Bending forward will worsen your pain."

5) An ordinary physical therapist makes you dependent on their services due to a lack of education and communication skills.

6) An ordinary physical therapist follows outdated approaches and interventions, claiming they can "release your fascia" with their hands, "regenerate degeneration," or that electrotherapy is actual physical therapy.

I’ll share "How to identify a competent physical therapist" in the next post.

If I could, I would remove outdated electrotherapy and passive therapies from the DPT curriculum.

What Manual therapy can and can't do. "Let's clear things up"
There are bunch of narratives claim by Physical Therapist, Most of them are pure nonsense.

If your surgeon says that your only options for your low back pain or disc bulge are surgery, drugs, or injection therapies.

Change your surgeon, you’re on the wrong station.

If your physiotherapist told you that your spine is misaligned, if they told you that they will release your fascia, If they blame posture for every thing, If they told you that you have to SIT UP STRAIGHT for 8 hours, If your treatment consists of more time on electrotherapy and modalities than on active rehabilitation, If they perform the same exercises on you daily, or if they don’t educate you about your problems and how they can help you fix them,
RUN. CHANGE YOUR PHYSIOTHERAPIST. YOU'RE ON THE WRONG TRACK.

I have upper back pain since few weeks. My junior convinced me to get an ultrasound on my T-spine yesterday, and I felt better for a while, this might be due to the ultrasound probe rubbing on my back. Regardless, my main point is that your greatest asset as a physiotherapist is your ability to communicate. You can convince your patients to do their HEP, just as my junior convinced me to get an ultrasound.

Dear New Grad DPTs! ,

Many recent DPT graduates and students are doing internships in private clinics without having completed a house job or training in teaching hospitals. Unfortunately, many institutes lack affiliation with teaching hospitals, preventing them from providing standard supervised clinical practice and house jobs.

My advice to them is that while it’s fine to work as an intern in clinics, you will only observe or treat a limited range of cases in msk, neuro, or pediatric areas. It is essential to work in a teaching hospital to gain exposure in different wards and units, including pre and post-operative rehab, critical care physiotherapy in ICU and CCU, and dealing with complex patients, such as those with metastasis. You will learn to treat patients ranging from neonatal to geriatric while collaborating with other health professionals.

Remember, your opportunities are not limited to small clinics, your job could include from emergency care in the ICU to working with field athletes. After gaining this exposure, you can decide on your future specialty, whether it be sports and musculoskeletal physical therapy, strength and conditioning, neuro, critical care physiotherapy, pediatrics, pelvic floor physical therapy, and others.

Keep in mind that physiotherapy isn't just applying modalities like electrical stimulation, heat, ultrasound, and manual therapy in a private practice.

Sincerely,
Dr Physio

Dear Patient,

If your physiotherapy treatment consists of 10 minutes on TENS/ESTIM, heating, 10 minutes on ultrasound, a few minutes with the Theragun, cupping, Few minutes of STM, and a few minutes of manual therapy, but lacks pain and exercise education, and if your physiotherapist doesn't explain you "What's your problem and how they're going to help you fix it. CHANGE YOUR PHYSIOTHERAPIST.

Sincerely,
Dr Physio

READ THE CAPTION.

Today is World Physical Therapy Day. And while some may argue that there is a day for literally everything, I do think this field deserves a day to celebrate what it can do for people.

During my career as a physical therapist, I have been able to be a part of amazing things.

I was there to jump up and down and cheer for my patient who suffered a stroke while he took steps for the first time unassisted.

I taught a woman who had bilateral knee replacements how to get up and down from the floor so she could play with her grandchildren again.

I’ve helped ICU patients who have spent days laying on their backs sit in a chair to eat a meal upright.

I’ve worked with women postpartum who wanted to laugh, sneeze, or run again without peeing themselves.

I’ve guided athletes in their return to the sport that they love, that gives them purpose.

I took the first steps with a man who got his new prosthetic leg.

I came up with crazy obstacle courses for a woman with vertigo to complete to make sure she was able to care for her sick husband.

I’ve been a literal shoulder to cry on for patients in chronic pain that have been written off by the medical system as crazy or addicts.

I’ve heard wonderful stories told by patients’ family members about how happy they are their loved one can cook again. Can garden again. Can go on a hike again.

This post isn’t to brag about the things I have done, by any means, because it truly is in a days work of being a physical therapist, and there are so many other stories PTs could tell of their best, hardest, saddest, and most triumphant patient encounters. This post is to brag about what an incredibly bad ass profession I am a part of, and how proud I am that physical therapy helps people all over the world, of every age, gender, and race, achieve things they didn’t think they could do. That, my friends, is pretty damn amazing.
