Hereโ€™s the gist:

1. If a patientโ€™s HbA1c is high (like above 9%) but there are ๐™ฃ๐™ค ๐™จ๐™š๐™ง๐™ž๐™ค๐™ช๐™จ ๐™จ๐™ฎ๐™ข๐™ฅ๐™ฉ๐™ค๐™ข๐™จ ๐™ค๐™ง ๐™ค๐™ง๐™œ๐™–๐™ฃ ๐™™๐™–๐™ข๐™–๐™œ๐™š, start with oral meds (metformin first, then combine as needed).
2. If thereโ€™s ๐™๐™š๐™–๐™ง๐™ฉ ๐™™๐™ž๐™จ๐™š๐™–๐™จ๐™š ๐™ค๐™ง ๐™ค๐™ง๐™œ๐™–๐™ฃ ๐™ž๐™ฃ๐™ซ๐™ค๐™ก๐™ซ๐™š๐™ข๐™š๐™ฃ๐™ฉ, keep monitoring closely โ€“ if no improvement after a few months, then consider insulin.
