Gastroduodenal Obstruction – A Common Presentation in Upper GI Cancers

Gastroduodenal obstruction is frequently seen in patients with upper gastrointestinal tract malignancies, where tumors compress the gastroduodenal lumen either from inside or outside, leading to blockage.

Common Symptoms Include:
▪ Persistent vomiting & nausea
▪ Severe weight loss
▪ Abdominal or chest pain due to reflux
▪ Anemia
▪ Aspiration pneumonia

👉 The main challenge in management is **restoring and maintaining lumen patency** so the patient can resume an oral diet.

Endoscopic Stenting – Key Considerations
The procedure is generally straightforward, but requires precise knowledge of:
Guidewire diameter
Working channel size
Appropriate stent selection

Procedure Tips:
🔹 Decide between Through-The-Scope (TTS) or Over-The-Scope (OTS) placement
🔹 Measure stricture length accurately using fluoroscopy before choosing stent length

TTS Approach (Relatively Easier):
✔ Therapeutic gastroscope with **3.7 mm working channel**
✔ Guidewire 3.5–3.8 Fr
✔ Slim stents (e.g., Bona Stent) designed for TTS

⚠️ Standard adult colonoscopes (3.2 mm channel, 2 m length) usually do not work
✔ CF260i / CF260T scopes may be suitable due to shorter length & wider channel

OTS Approach:
✔ Adult or pediatric gastroscope
✔ Guidewire 3.5–3.8 Fr
✔ Commonly available duodenal stents

⚠️ Esophageal stents are not ideal due to rigidity and poor angulation fit
⚠️ Colon stents are larger but may be used selectively

📌 In this case, initial OTS placement failed, and successful stenting was achieved using the TTS approach.
