Successful Emergency Management of Upper GI Bleed

A 62-year-old patient with known ischemic heart disease presented with fresh hematemesis, resulting in a severe drop in hemoglobin to 4 g/dl.

Urgent upper GI endoscopy revealed a large longitudinal ulcer located at the esophagogastric junction, a technically challenging area, with active oozing of blood. A visible vessel was identified at the lower end of the ulcer.

Mechanical hemostasis was achieved using a hemoclip, followed by heater probe coagulation at 35 Joules. Complete hemostasis was confirmed at the end of the procedure.

The patient received blood transfusions, responded well to treatment, and was discharged safely. Cardiology follow-up was advised for further management of antiplatelet therapy.

Fundal Varix Sclerotherapy done with 2cc B-Braun Cyanoacrylate

Indication:
Evaluation of upper gastrointestinal bleeding / Portal hypertension

Procedure:
Diagnostic and Therapeutic Upper GI Endoscopy

Findings:

Esophagus: Multiple columns of small-sized, non-bandable esophageal varices observed.

Stomach: Marked portal hypertensive gastropathy seen throughout the gastric mucosa.
A large isolated fundal varix with red sign was identified along the greater curvature.
Injection sclerotherapy performed using 2 cc B. Braun Histoacryl; adequate hemostasis achieved.

Duodenum: Severe duodenopathy noted; no evidence of ectopic varices.

Impression:

Multiple small non-bandable esophageal varices

Severe portal hypertensive gastropathy

Large isolated fundal varix (treated with Histoacryl injection; hemostasis achieved)

Severe duodenopathy

Plan / Recommendations:

Continue non-selective β-blocker (e.g., carvedilol) for portal pressure reduction

Monitor for rebleeding and hemodynamic stability

A patient presented with persistent high-output biliary drainage following cholecystectomy.

ERCP for Biliary Pancreatitis/ Choledocolithiaisis

Procedure Details

Scope advancement: Duodenoscope was advanced up to the second part of the duodenum (D2).

Papilla: Papilla identified and appeared edematous.

Cannulation: Selective cannulation of the common bile duct (CBD) achieved using guidewire-assisted sphincterotome technique in the first attempt.

Cholangiogram Findings:

Dilated common bile duct with a large filling defect in the distal CBD consistent with a stone.

Intrahepatic biliary radicles mildly prominent.

Therapeutic Intervention:

Adequate biliary sphincterotomy performed.

Multiple balloon sweeps performed using extraction balloon.

A large CBD stone retrieved; subsequent sweeps yielded clear bile.

Occlusion Cholangiogram: Confirmed clearance of CBD with free flow of contrast into the duodenum.

Pancreatogram: Not attempted.

Complications: None encountered during or after the procedure.

Impression: Successful ERCP with complete CBD stone extraction in a case of choledocholithiasis/biliary pancreatitis.

🩺 Successful ERCP – Distal CBD Stone Extraction

A case of obstructive jaundice was managed successfully through ERCP.
✅ D2 approached and selective cannulation of CBD achieved in the first attempt using a guidewire and sphincterotome.
✅ Cholangiogram revealed a large filling defect in the distal CBD causing gross dilatation of proximal CBD and IHBD.
✅ Ample sphincterotomy performed, resulting in abrupt evacuation of a huge CBD stone.
✅ Subsequent balloon sweeps cleared the bile duct completely.
✅ Occlusion cholangiogram confirmed no residual filling defect.
✅ Jaundice effectively relieved.
⏱ Total procedure time: 10 minutes.

📌 PEG Tube Insertion in a Patient with Oropharyngeal Cancer

Successfully Percutaneous Endoscopic Gastrostomy (PEG) tube insertion in a patient suffering from cancer of the oropharynx who was unable to maintain adequate nutrition due to severe swallowing difficulty.

🔹 Under endoscopic guidance, the PEG tube was safely implanted directly into the stomach.
🔹 The procedure was smooth and well tolerated.
🔹 This feeding tube will now allow the patient to receive proper nutrition and medications, ensuring strength and energy during cancer treatment.
