A patient underwent a total knee replacement on the left side in 2020. Over the past 6-8 months, they have experienced pain while walking, along with limited knee flexion. The range of motion is restricted beyond 80 degrees and is painful. Despite this, muscle strength is intact, with both the quadriceps and hamstrings functioning well, and no contractures present. The orthopedic surgeon has now recommended a revision total knee replacement. What could be the underlying reason for this recommendation?

PARAPLEGIA

Paraplegia is a condition characterized by paralysis or loss of function in the lower half of the body, including the legs and possibly the trunk. It typically occurs as a result of damage to the spinal cord, most commonly from injury or trauma, such as a spinal cord injury (SCI). Paraplegia can also be caused by medical conditions such as spinal cord tumors, infections, or autoimmune diseases like transverse myelitis.

The extent and severity of paralysis in paraplegia depend on the level of the spinal cord injury or dysfunction. Injuries higher up on the spinal cord can result in more extensive paralysis, while injuries lower down may allow for some movement or function below the level of the injury.

Common symptoms and complications associated with paraplegia include:
1. Loss of sensation and movement in the legs and lower body.
2. Difficulty or inability to walk, stand, or bear weight on the legs.
3. Loss of bladder and bowel control, requiring the use of catheters or other interventions.
4. Sexual dysfunction, including erectile dysfunction and loss of fertility.
5. Muscle spasticity or stiffness in the legs.
6. Pressure sores (bedsores) due to immobility, requiring careful skin care and positioning.
7. Secondary health issues such as respiratory problems, osteoporosis, and urinary tract infections.

Management of paraplegia typically involves a multidisciplinary approach, including medical treatment, rehabilitation, assistive devices (such as wheelchairs or braces), physical therapy, occupational therapy, and psychological support. The goal of treatment is to maximize independence, improve quality of life, and prevent complications. Advances in medical and rehabilitative care have significantly improved outcomes and quality of life for individuals living with paraplegia.

Office Syndrome

Office Syndrome caused by the use of certain muscles incorrectly according to the body or Using the same muscles repeatedly For a long time continuously, such as tensing the neck, sitting in front of the computer for a long time Lifting things in the same position for a long time, etc.

cause of office syndrome is when the muscles in the neck, shoulders, and shoulders contract and do not relax. until it becomes a lump (trigger point), causing the blood and oxygen to be unable to circulate easily The patient will begin to have pain or stiffness in the neck, shoulders and shoulders, and if left without proper treatment. The body will begin to create a thick membrane to cover up. As a result, the muscles in that area become inflamed, causing more pain.

If the fascia that attaches to the adhesion begins to spread to squeeze the surrounding nerves Patients will begin to have symptoms of abnormalities in that nerve, for example, they may have pain that spreads up to the ear, jaw, jaw, eye socket, head, or spreads down the arms and chest, causing chest tightness, shortness of breath, and difficulty breathing for unknown reasons.

If not treated properly Office syndrome can develop into Cervical sciatic nerve condition This is because the fascia will begin to hold on and pull back to the cervical vertebra in that area. deformed and to crush nearby nerves This results in pain in the arm, numbness in the arm, and eventually weakness in the arm.
